On 06/ 24/2024 at 09:00 AM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced 3 Year Annual Required Inspection. Facility is located at 21203 Devonshire Avenue, Granada Hills, CA 91311. LPA identified self and met with Mercedes Diaz, Director LPA identified reason for visit and provided Entrance Checklist -Child Care Centers (LIC 125) to help facilitate the inspection.

Facility Representative (FR) Mercedes Diaz, Director allowed entry to facility and provided LPA a guided tour of inside and outside of the facility. The facility is currently licensed for a capacity of 46, per FR they currently have 5 enrolled. The facility hours of operation are currently Monday thru Friday from 06:30 AM to 06:00 PM.

Required postings were observed posted in lobby and in each classroom.

The facility is made up of two infant classrooms: Infant Room #1 ages 6 weeks to 1 year and 1 staff present with 1 child. Infant Room #2 ages 1 year to 2 years old, and 1 staff present with 1 child.

Beginning at 09:10 AM, LPA observed the following in each classroom. The furniture, toys, and equipment were inspected for age appropriateness and were in good repair. There were cubbies used for each child to store their belongings. In Infant Room #1, there were cribs with tight fitted bed linens provided by families and taken home weekly for washing with mattresses fitted for the crib, there was gate separating the sleeping area from play area and line of vision is maintained from the front to the back of the room. In Infant Room #2, there were standard blue cots and bed linens provided by the families and taken home weekly for washing. Drinking water was readily available and sippy cups were identified with each child’s name. The restroom(s) were inspected for safety and sanitation. There were age-appropriate sink and toilet in Infant Room #2 at this time all children enrolled are using diapers. There is telephone service via landline, heating, lighting and ventilation were evaluated for safety and comfort.

LPA observed first aid kits in each classroom and inventoried for necessary supplies. LPA observed a device high on the ceiling per FR it is a carbon monoxide detector. LPA observed smoke detector via fire alarm. Disaster drill log was available. Per FR, isolation area for sick children in Infant Room #1 and #2 is to move the child to the front or back of the room away from other children and there is an area available to lay down. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPAs observed cleaning solutions stored under sink cabinets with a child proof safety latch and high shelves in each classroom.

Beginning at 09:44 AM, LPA observed the kitchen area/ food preparation area for safety, cleanliness, and proper equipment. Facility has families provide lunch and prepares snacks and drinks and snack menus were reviewed and posted in each classroom and lobby. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair.

Beginning at 09:23 AM, LPA toured the outdoor play area and observed playground equipment in safe condition, free of sharp, loose, or pointed parts, in good repair and age appropriate. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. The children have access to water from the classrooms that is brought to the play area for drinking water during outdoor play. LPA advised that no children shall be left without the supervision at any time. There is no swimming pool or other bodies of water on the premises.

Beginning at 09:50 AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 06/17/2025 for Mercedes Diaz, Director. Staff files reviewed did not have proof of the AB 1207 Mandated Reporter Training certificate on file due to Staff #1 has 90 days from 05/24/2024 to complete. Inspection of required forms were discussed.

LPA observed that teacher-child ratios were maintained and staff names were recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed 6 of 7 children signed in at time of inspection. Personal Rights of children were observed by LPA.

This facility does not currently provide Incidental Medical Services - IMS. LPA discussed Incidental Medical Services (IMS) policy with Director. For IMS information see PIN 22-02- CCP. Facility Representative was reminded that all adults 18 and over, including
employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND IS IN THE PROCESS OF REMEDIATING LEAD EXCEEDANCES
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with licensee facility representative and

LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00.
