On 02/01/2024 at 10:40am, Licensing Program Analysts (LPA) Sarah Garcia and Cristina Castellanos conducted an unannounced case management-incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 01/24/2024. Upon arrival, LPAs met with Director, Demi Lara and assistant director, Aisha King. LPAs informed director about the purpose of the visit and toured the facility. LPAs observed 74 children with 8 staff.

LPAs observed classrooms:

Preschool 1: 23 children with 3 staff

Preschool 2A:19 children with 2 staff

Preschool 2B: 21 children and 2 staff

Preschool 3: 11 children with 1 staff

According to the UIR, on 01/24/2024, Child 1 (C1) was found in the pre-school 1 classroom without teacher supervision after the classroom transitioned from using the restroom to the outdoor area. Ms. Marie found C1 in the classroom alone. C1 stated they were using the restroom. LPA observed a purple exit door leading to the parking lot of the center. LPAs took photographs of the door. The director stated that the center was licensed that way and another analyst did not mention anything about it. LPA consulted LPM Maureen Neal and will gather additional information regarding the door.

LPA Garcia and Castellanos conducted interviews with the director, staff, C1 and received the following documents: children’s roster, personnel roster, parent incident notification, teacher plan of corrections, and witness statements. Based on interviews and evidence provided, physical plant inspection of the site of the incident, it revealed that at the time of the incident, there were 18 children playing outdoors being supervised by two staff members. Per interview with preschool director, on 01/23/2024 at approximately 10:40 AM, a teacher observed C1 unattended in the classroom when transitioning the children from bathroom to outdoor time. The preponderance of evidence to prove there was a lack of supervision has been met. A type A citation was issued. An immediate civil penalty of $500 will be assessed.

Exit interview was conducted and a copy of the report was provided to director, Demi Lara. Appeal rights were reviewed and provided.

LPAs Garcia and Castellanos informed Director, Demi Lara that this report dated 02/01/2024 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.



Also, LPAs Garcia and Castellanos informed the Director, Demi Lara to provide a copy of this licensing report dated 02/01/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
