Licensing Program Analyst (LPA), V. Wheatley met with the applicant, Ndidi Duruhesie at 11am for the purpose of conducting a prelicensing. The applicant is licensed currently at the same location however is requesting change of ownership due to a new corporation. The applicant is requesting a license for 25 preschool children.

LPA and applicant followed Los Angeles County Guidelines in regards to Covid-19. LPA observed some Covid-19 signs posted at the entrance, health screening and temperature check for adults and separately for children. The staff and children will wear face mask and stay six feet socially distant. LPA also observed sign in and out procedures.

LPA observed adequate lighting and ventilation in every classroom. Age appropriate toys and equipment were observed in good repair. The children are utilizing their own separate toys at this time. No pillows, stuffed animals and the like are allowed at this time. LPA observed cubbies and adequate storage for the children's belongings. LPA observed napping equipment stored separately. The bedding/linen is kept separate.

FOOD SERVICE - The food will be cooked on the premises. The children will be served breakfast, lunch and snack individually. The kitchen is off-limits. LPA observed extra food, water bottles and utensils. LPA observed cleaning supplies and disinfectant which is kept out of the reach of children. LPA observed a first aid kit. Drinking water will be provided for children to freely drink. The ill children will be kept in the school office for isolation and the staff restroom is adjacent.

The facility was inspected by the Los Angeles County Fire Department and granted a fire clearance for 25 preschool children/ capacity. The facility is equipped with fire extinguishers, carbon monoxide detectors and smoke detectors. The staff will conduct fire drills and earthquake drills regularly. The drills will be logged.

  • The "Notification of Parent's Rights" (PUB394) was discussed with the licensee and the licensee was advised that it must be posted in an area of the home accessible to parents.
  • Children's forms will be completed by authorized representative prior to enrollment and kept separately.
  • Applicant was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects to disseminate information on the State’s licensing role, provide information to the public and parents on childcare licensing, and provide many other helpful resources to the licensees and the public.   • Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. Exemption were also discussed.
  • Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.   • Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.   • Child Care Quarterly updates will be read as they come out, to stay informed of any changes or updates to statutes and regulations.
  • Unusual incidents will be reported to the department within 24 hours and written on LIC 624 within 7 days.
  • LPA Wheatley discussed AB633 and informed applicant that upon receipt of a Type A deficiency, the applicants shall post and provide copies of this licensing report (LIC 809 or LIC 9099 and copy of LIC 9224) to parent/guardians of children in care at the facility and to new parents/guardians of children enrolled at the facility during the next 12 months. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed.

INDOOR ACTIVITY SPACE

LPA Wheatley measured three preschool classrooms as follows:
Classroom 1 at entrance measured 247 square feet.
Classroom 2 measured 140 square feet
Classroom 3 (adjacent to the children's restroom) measured 386 square feet
Indoor Total = 773 square feet = 22 capacity

OUTDOOR ACTIVITY SPACE- LPA inspected the Outdoor Activity Space which is a large yard and patio.

Large yard - 2,080 square feet
Covered patio - 416 square feet
Outdoor Total = 2,496 square feet = 33 capacity

Based on today's inspection, a license will be granted for 22 when the following corrections are completed and submitted to the department:



1. The wood on the garage facing the yard to be replaced and made safe.
2. Install latch on closet door in classroom.
3. Make the wires from television inaccessible.
4. The water hose will be made inaccessible.
5. The emergency disaster plan will be updated.
6. Additional Covid-19 signs will be posted at the entrance.

An exit interview was conducted. A copy of this report will be provided to the applicant by email via read receipt.
