13 | On 07/10/2023/ LPA Whitmore initiated the complaint investigation and met with the Director Felecia Keene. LPA toured the facility indoors and outdoors, observing proper teacher/infant ratios with a total of 18 infants and 5 teachers. LPA conducted a one-hour observation of the infant room and napping area. After interviewing the Director and Staff, LPA obtained a copy of the Facility Roster, Personnel Report, Agenda, Sign in Sheets from meeting, Care and Supervision Policy and Incident Reports.
On 09/27/2023 at 1:54p.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Licenee/Director Felecia.LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 18 total children in care and 6 Teachers.The Department conducted a full investigation, which included staff interviews, interviews with relevant parties, as well as a record review which included documentation related to the allegations. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Personal Rights-Day Care children sustained unexplained injuries while in care. And for Neglect Lack of Supervision to children in care-facility staff are not providing adequate supervision to children in care.
