An unannounced Case Management-Incident inspection was conducted on this date by Licensing Program Analyst (LPA) Amelia Morales to follow up on an Unusual Incident which occurred on 2/07/25 and was reported via phone on 2/10/25 to Community Care Licensing. LPA Morales was greeted by Lead Teacher Nixie Ramos Lopez. LPA Morales stated the purpose of this visit, Lead Teacher Nixie Ramos informed LPA Morales that the Director stepped out for a moment but would be back shortly. Lead Teacher Nixie Ramos Lopez guided LPA Morales on a tour of the facility, census was taken. There were 7 toddlers and 2 staff present at the time of the visit. The Director Petrena Wisdom arrived shortly after, LPA Morales informed Director Petrena Wisdom the purpose of the visit

The incident that occurred on 02/07/25 was reported to the Department via telephone on 02/10/24. The Unusual Incident which was reported, had concerns over blood in diaper on Child (C1).

LPA Morales was given a DCFS referral number by the Director, took photos of the photo of the blood on the diaper, took a photo of the discharge from UCLA that is dated 2/10/2025, and obtained phone numbers.

During the inspection conducted on March 21, 2025 LPA Morales conducted interviews with the Director and one staff. When asked what exactly occurred, the Director informed LPA Morales that during the facilities routine first diaper check staff one observed pink blood on (C1) diaper. The Director called 911, DCFS, as well as the ambulance, (C1) was taken to the hospital and was examined. (C1) was discharged and was able to return back to the facility on February 11,2025.
