Licensing Program Analyst (LPA) Denise Miranda conducted an unannounced complaint inspection to the above facility on 03/23/2023. LPA arrived at the facility at 12:35PM and met with Darlene Davis-Ayala, Director, who guided analyst on a tour of the facility. There were 14 children with 02 staff upon arrival.
During the investigation LPA interviewed staff, interviewed parents and children. LPA obtained a copy of the children's roster, and obtained copies of supporting documentation.
Information provided by the reporting party indicates that Staff do not have the required education to provide care, Staff are not following proper food- service sanitation practices and Staff do not ensure that linens are sanitary.
LPA reviewed staff#1, Staff#2 and Staff#3 files, and observed that all the staff have the required education to provide care.

When interviewing staff, Staff #1, #2 and #3, disclosed that Staff are following proper food- service sanitation practices and they also ensure that linens are sanitary.
Parents interviewed disclosed that Staff are following proper food- service sanitation practices and ensure that linens are sanitary.
LPA Miranda interview, Child#2 and Child#14, that confirmed the teachers changing the children’s diaper at the bathroom and did made not disclosures regarding the other two allegations, and LPA attempted to interview Child#10, Chid#11, Child#12, Child#13 and Child#14, however, children made not disclosures regarding the above allegations.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Darlene Davis-Ayala, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
