On 2/8/2024 Program Analysts (LPA), Judy Laureano and Rantia Richmond conducted an unannounced Annual Required Inspection at above mentioned facility.
Upon arrival, LPAs were greeted by J. Duan, staff. She mentioned that licensee Maximillian Getmanchouk was out taking his grandmother to the doctor.

Present during today’s inspections was individual Dmitri Kondrashov, who is a licensee at another home. LPAs asked D. Kondrashov why he was at this facility, he confirmed he only works here in the morning and is at his facility from 11:00 a.m. to 5:00 p.m.
D. Kondrashov, confirmed that he is the stepfather to licensee. D. Kondrashov also confirmed that Licensee is currently working. LPA Richmond asked D. Kondrashov the licensee’s work schedule and he instructed LPA to contact Licensee directly. LPAs contacted licensee at 10:17 a.m. and requested a call back. LPA issued a type B citation.

LPAs toured the home inside and outside. LPA observed 5 children in care; with one adult; J. Duan. LPA verified facility roster via Guardian.

The hours of operation are Monday and Friday 8:00 a.m. to 5:30 p.m. Currently facility is available to take children ages 18 months to 3 years old. Facility is licensed for a Small Family Child Care license with a max capacity of 8 children.

Home is a two story with 1 bedroom, bathroom, kitchen and dining with a loft area. LPA observed presenter’s badges, election mail and medication belonging to D. Kondrashov.

The first floor has a living room ,dining room, kitchen, 2 bedrooms, 3 baths, 1 den living room an second den with a storage room and an attached garage.

Staff confirmed the following areas are used for the day care: Living room, dining room and bedroom 1/napping room and bathroom. Licensee was informed that any changes to ages, hours and days of operation shall be submitted to the department via signed LIC 279, for approval prior to initiation of changes.

Staff J. Duan confirmed the following areas are OFF LIMITS to the children in care: Bedroom two, observed with a doorknob cover. Bedroom two gives access to the rest of the home that has been designated as OFF LIMITS to the children.

Outdoor was observed in the front yard. LPA observed an age appropriate outdoor equipment. LPAs observed the bottom of the step cracked, a potential hazard to the children in care- LPAs issued a Type B citation.

LPA observed the area designated for day care use and observed age-appropriate toys. Licensee confirmed children nap in bedroom 1, LPAs observed napping cots.
There no firearms or ammunition on the premises. Detergents and cleaning compounds are under the sink cabinet. LPAs observed a First Aid Kit in the dining room area.
Homes does not have any child enrolled that might need medication.

LPAs observed licensee test the smoke detector in the home. LPAs did not observed a working carbon monoxide detector and a Type A citation was issued. A working fire extinguisher was observed in the home. Licensee confirmed that fire drills and earthquake drills are completed. Log was not available for review, Technical Violation was issues. Each family child care home shall conduct fire drills and disaster drills at least once every six months. The licensee shall document the drills, including the date and time of each drill. This documentation shall kept at the family child care home.

Families and licensee provide meals and snacks. LPA discussed the importance of maintaining a system where allergies and food restrictions are noted. LPA encourage licensee to contact their local resource and referral agency, Crystal Stairs, to inquiry about the different resources and professional development opportunities available, including local food program.

Adequate heating and ventilation for safety and comfort were observed in the space. Safe toys and play equipment were observed. The home has a working telephone service and LPAs confirmed the phone number and email address.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children.

Capacity as specified on the license is being maintained during today’s inspection; 5 children were present during today’s inspection.

Licensee’s Mandated Reporter was taken on 3/1/2022. Licensee’s Pediatric CPR and Pediatric First was taken on 3/102/2022. LPA reminded licensee the importance of making sure all vendors providing Pediatric CPR and Pediatric First Aid need to be EMSA approved. LPAs reviewed 6 children’s file and files were complete. LPAs discussed all necessary forms needed in each staff file and children’s file. LPAs provided licensees with a current copy of the LIC 311D and LIC 126 to use as a reference when auditing files; documents were emailed prior to visit and hard copies were provided during today’s inspection.

Facility representative was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


A notice of site visit was given and must remain posted for 30 days.
