On 07/30/2025 Licensing Program Analyst, LPA, Tyra Chavies, met with assistant director, Elizabeth Cooper and licensee,Shirani Perera, for the purpose of an unannounced Case Management visit to deliver complaint findings. There were 47 children being supervised by 4 staff members.

07/21/25 LPA Chavies conducted phone interviews with S#7 and RP
07/18/25 LPA Chavies attempted to conduct interviews with S#7 and S#8
06/25/25 LPA Chavies received via email updated personnel document
06/22/2025 LPA Chavies received via email children's facility roster, personnel documents and LIC 624
06/17/25 LPA Chavies observed children in care and conducted in person interviews with S#1, S#2, S#3, S#4, S#5, S#6, Licensee and Director

Based on LPA's observation, record review, interviews conducted, the preponderance amount of evidence standard has been met.

Therefore, the above allegation, Staff yelled at daycare children is found to be SUBSTANTIATED. Per California Code of Regulations Title 22, Division 12 Chapter 1, this facility is being cited a Type B. (Please see LIC 9099D.)

An exit interview was conducted with assistant director and licensee this report was read and a copy of this report was given.

Notice of site visit was provided and must be posted for 30 days.
