On June 6, 2025, Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced Annual/ Random Inspection for the Village Park Montessori Preschool license. During today’s inspection, LPA met with preschool assistant director Elizabeth Cooper, director, Yeshenia Garzon, and licensee, Shirani Perea toured the facility indoors and outdoors.  Days and hours of operation are Monday through Friday from 6:30 AM - 6:00 PM. LPA Chavies confirmed the phone number is: (424) 257-8075. There were 48 children in care being supervised by 8 staff members.

This facility currently has three classrooms Caterpillars, Butterfly and Dolphins, a child's library, an indoor playground and 2 outside playgrounds for the preschool license. All playgrounds and classrooms are currently being used. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. LPA observed cots for sleeping and bedding will be given to the parents at the end of the week and parents will bring bedding back on Monday. LPA also observed age appropriate toys and books in primary care areas. LPA observed over the counter medication (diaper rash creams and sunblock).

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed 10 or more over the counter medication that was not properly stored and some of the medication was expired. LPA advised director and licensee that over the counter medications is to be stored separately, in a bag with a note from parents/ guardian and/or doctor with instructions. LPA observed assistant director, discard the expired over the counter medication. This facility does not provided any meals an/or snacks, parents are required to provide Breakfast, lunch and AM/PM snacks. LPA advised director and licensee that the facility must provide an midmorning snack and an afternoon snacks per regulations.

The surface of the outdoor activity space is maintained in good condition and is free of hazards. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area.

LPA inspected and verified that there is a fire system connected throughout the facility. The facility does have a fire extinguisher which has an inspection date of April 24,2024 on the premises. LPA was advised by director, the fire department will be at the facility on June 11,2025 to conducted another fire extinguisher inspection. Facility has one or more functioning carbon monoxide detectors. First Aid kit was observed and is in good condition. Drinking water is available both indoors and outdoors and upon request. This facility is not required to have it's water tested for lead. Parents can bring their children water bottles or the facility must provide water to the children upon request.

POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall (Facility Sketch, PUB 394, LIC 9148, LIC 610A). Fire and disaster drills are being conducted as scheduled every six months.

CHILDREN'S RECORDS: LPA Chavies reviewed 10 children's files. Most of the documents were completed. LPA Chavies advised director to conducted a file review to ensure all documents are completely filled out. LIC 955 is missing for most/ all children's records. LPA observed sign in/out book. LPA advised director that parents/guardian must sign in and out at the appropriate time and must ensure the parents are signing the child in and out of the facility on a daily bases.

STAFF'S RECORDS: LPA Chavies reviewed 4 staff files. One or more files are missing the following: mandated reporter training, Education qualifications, immunizations records and LIC 503 form. LPA advised director that a person cannot be employed or volunteer at a childcare facility or family day care if he or she has not been immunized.

  • Licensee and director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

  • Beginning January 1st, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.
  •  A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.
When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN).
