On 11/10/2021 at 2:40 pm Licensing Program Analysts (LPA) Deborah Lowe met with applicant Zahra Haddadi. The purpose of the visit was a second announce pre-licensing inspection to review for an application at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no child present during this inspection. This is an application for 29 infants, ages 0 months to 24 months. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m.

Niaz Saee Haddadi was also present during visit. The fire clearance was approved on 09/16/2021.

The 1st pre-licensing visit was conducted on 10/21/2021. LPA Lowe is conducting a 2nd pre-licensing visit to evaluate the facility corrections needed before a license application will be considered.

  • Front windows – applicant stated window screens being replaced – LPA Lowe observed screens on the front windows in good condition.
  • Designate isolation area separate from child care areas – Isolation area has been observed, located in a small room near room 2. Isolation area was observed with a crib.
  • Walk though gates between play areas (3 in total) are in need of repair – LPA Lowe observed walk through gates with repairs made to the latching system. Doors were observed to fully close.
  • Air vent in room 4 located in the ceiling needs to be secure – LPA observed air vent to be secured.
  • Crib area near room 3 needs to be separate from activity area – LPA observed crib area with a half wall installed separating area from activity area. LPA advised applicant when infants are in crib area a staff member must be supervising the crib area from within the crib area.

  • Outdoor backyard area repair gate panels, remove debris, and ceiling fan – LPA observed outside area to be free of debris, and gate panels repaired.
  • Outdoor front play yard secure entrance gate – entrance gate was observed with hardware that locks with a key. Applicant stated front play yard gate will remain closed and locked during operating hours. Parents and staff will enter from the back entrance off of parking lot. LPA Lowe advised facility should place a sign on the gate reminding staff and visitors that the gate is to remain locked during operating hours.
  • Outdoor front play yard Gate surrounding outdoor area measures 3.2 feet – must be 4 feet or higher. LPA Lowe observed and extension to the gate making gate measure 4.3 feet. LPA Lowe observed 5.7 feet of the surrounding gate to still measure 3.2 feet.
  • Outdoor front play yard wires near hose area make inaccessible to children – LPA Observed enclosed bench as been placed over hose area making area inaccessible to children.
  • Replace first aid kit – First aid kit was observed to have bandages, tape, scissors, tweezers, first aid manual, antiseptic wipes, and band aids.
  • Display required postings – LPA Lowe observed the following required postings:
PUB 393 Notification of Parent’s Rights, LIC 613A, LIC 610 Emergency Disaster Plan, LIC 9148
Earthquake Preparedness, and Car Seat poster
  • Outdoor waiver request, include room schedules, outdoor schedule with 15 min delay between each group. Applicant provided LPA with supporting documents to request an outdoor waiver.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA Lowe provided applicant with a blank copy of LIC 9227 Individual Infant Sleeping Plan.

Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The following corrections are still needed before application will be considered.

  • Outdoor front play yard Gate surrounding outdoor area measures 3.2 feet – 5.7 feet of surrounding gate still measures 3.2 feet - must be 4 feet or higher.
  • Outdoor waiver letter request.
  • Outdoor waiver will need to be approved by Management.

An exit interview was conducted with the above items discussed and a copy of this report was provided to the Applicant Zahra Haddadi and Niaz Saee Haddadi. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues. Applicant to correct items by 11/15/2021.
