Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 1 year inspection. Upon arrival, LPA met with director, Zahra Haddadi. A COVID-19 risk assessment was conducted. LPA provided Director with a copy of the LIC 125 Entrance Checklist to facilitate the inspection. This is an Infant Program. Business hours are from Monday to Friday, 8:00 AM to 5:00 PM.

At around 8:30 am, LPA was guided to a facility tour by the director. All areas identified on the Facility Sketch were inspected. Facility has a second level. There is an office, break room, and an adult bathroom located upstairs on the second level.

LPA Garibyan toured the classrooms located at the facility. The facility has a total of 5 childcare rooms (three classrooms and two nap rooms). The facility uses an electronic sign in system through the Himama program and completes wellness checks of children upon arrival to facility. LPA observed ten infants in care with five staff (including director). Per director there are 18 infants enrolled. Teacher child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license.


LPA observed crib area with a half wall installed separating area from activity area. LPA advised director when infants are in crib area a staff member must be supervising the crib area from within the crib area. Designate isolation area separate from child care areas and located in a small room near room 2. Isolation area was observed with a crib. LPA observed outside area to be free of debris.

Outdoor entrance gate was observed with hardware that locks with a key. Director stated front play yard gate remains closed and locked during operating hours. Parents and staff enter from the back entrance off of parking lot. LPA observed a sign on the gate reminding staff and visitors that the gate is to remain locked during operating hours. Outdoor front play yard Gate surrounding outdoor area measures 4.3 feet. LPA Observed enclosed bench has been placed over hose area in outdoor front play yard making area inaccessible to children. First aid kit was observed to have bandages, tape, scissors, tweezers, first aid manual, antiseptic wipes, and band aids. LPA Garibyan observed the following required postings: License, PUB 393 Notification of Parent’s Rights, LIC 613A, LIC 610 Emergency Disaster Plan, LIC 9148 Earthquake Preparedness, Car Seat poster, Outdoor waiver and outdoor schedule. LPA observed 5 carbon monoxide detectors and Fire Alarm system in the facility. Director tested carbon monoxide detector in room 3 and LPA were able to hear a successful test. Facility has working land line on the premises (818-824-3655).
All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard were observed to be free of obstruction. The child care center was observed be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Floors of all rooms have a surface that is safe and clean. A comfortable temperature for children was observed to be maintained. No pools or bodies of water was observed at facility. The facility was observed to be equipped with appropriate furniture and equipment including, but not limited to, cribs, cots or mats; a changing table; and feeding chairs. Furniture was observed to be maintained in good repair and safe condition. The type, height and size of furniture and equipment was observed to be age appropriate. All play equipment and materials used by infants was observed to be age-appropriate. Toys are safe, and do not have sharp points or edges or splinters, or made of small parts that can be pulled off and swallowed.

LPA observed individual permanent space for child’s clothing, personal belongings and/or bedding. All materials and surfaces accessible to children, including toys, was observed to be free of toxic substances.

Director stated facility does not provide the food, all food including snacks is provided by the parents. LPA advised all food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. LPA advised all foods or beverages capable of supporting the rapid and progressive growth of microorganisms that can cause food infections or food intoxication shall be stored in covered containers at 45 degrees F (7.2 degrees C) or less.

Director stated that prior to the infant's first day at the center, the infant care center director completes a needs and services plan for the infant. High chairs are used to seat infants during feeding. LPA observed infants who are unable to sit unassisted in a high chair or other seating equipment held by a staff for feeding. LPA observed bottles, dishes and containers of food brought by the infant’s authorized representative be labeled with the infant’s name and the current date.

Medications are kept upstairs the director's office inaccessible to children. The refrigerator in break room is used to store any medication that requires refrigeration. Director stated only prescription medication is allowed.

LPA advised the licensee shall develop and implement a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

LPA advised smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). LPA advised a baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c).

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. Pediatric First Aid/CPR reviewed and is current.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE. LPA referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

A notice of site visit was given and must remain posted for 30 days.
