On 02/19/2025, Licensing Program Analyst (LPA) Ranita Richmond conducted an unannounced visit to the above-named Home to conduct the Required 3-year Inspection. LPA was greeted by licensee Byeongim Park. LPA Richmond observed 4 children, being supervised and cared for by licensee. Hours of operation are Monday through Friday, 8:00am – 6:00pm. Licensee provides meals, snacks, and water. Korean translation by translator ID # 15727.

LPA Richmond toured the home inside and outside for a Health and Safety inspection. The home is neat and clean with heating and ventilation for safety and comfort.

LPA Richmond confirmed that the home is a two story home. On the top level upstairs is four(4) bedrooms and three (3) bathrooms. On the ground level, the home consists of living room, dining room, kitchen, 3 bedrooms, 1 bathroom, converted enclosed patio including lobby and two (2) activity play rooms, fenced front yard, laundry room, and fenced back yard.

The ON LIMIT AREAS are as follows: converted enclosed patio, which includes 2 activity play rooms and lobby area, bedroom #2 (napping/main daycare room/eating area), bedroom #3 (infant room), bathroom, and fenced front yard.

The OFF-LIMIT AREAS are as follows: entire 2nd floor, which includes 4 bedrooms and 3 bathrooms, kitchen, dining room, living room, master bedroom (bedroom #1), and gated back yard.

Children and parents enter through the front of the home entrance. Upon entrance you are in the converted covered patio, lobby area. To the immediate right and left of the patio are activity play rooms. Through the patio, straight ahead you will enter a wooden door to the original home layout. To the immediate right is the master bedroom (off limits), next to the master bedroom is bedroom #3 (infant room), inside of bedroom #3 there is a double door that leads to the living room. Inside the original home layout upon entering to the left is bedroom #2. In the entry way of the home is a barricaded staircase next to bedroom #3 on the right side that leads to the upstairs off limits area of the home that includes 4 bedrooms and 3 bathrooms. In the entryway of the home on the left next to bedroom #2 is a hallway. Straight ahead in the hallway is the bathroom. In the hallway to the right is the kitchen. Inside the kitchen to the right is the dining room. In the dining room there is a door that leads to the living room. Inside the kitchen if you enter and walk straight ahead you will see a door that leads to the fenced back yard. To the back of the kitchen just before the back door to the left is the laundry room.

Licensee is aware that the children must nap and have meals in the home.

LPA Richmond observed a fully charged 2A:10B:C Fire Extinguisher in the kitchen, 4A 60B:C fire extinguisher in the converted patio and working smoke detector and carbon monoxide detector in the hallway.

There are no firearms or ammunition on the premises.

There are no pools, ponds or other bodies of water on the premises.

LPA Richmond observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts.

During walkthrough LPA observed Raid and Cutter insect repellant on the front porch accessible to children in care. Type B citation issued. See LIC 809D. Upon entering the converted patio, LPA observed Off insect repellant and sunscreen on an open shelf accessible to children in care. LPA observed medications such as antiseptic creams on a mobile cart. Type B citation issued. See LIC 809D. LPA observed access to upstairs accessible to children in care as barricade was not positioned to make stairs inaccessible to children in care. Type B citation issued. See LIC 809D. LPA observed access to kitchen available to children in care as child gate was not closed making the kitchen accessible to children in care. Inside the kitchen LPA observed knives on the counter, medicines on the counter, and other disinfectants accessible to children in care. LPA observed beauty supplies and hygiene products such as hair dyes and mouthwash in the bathroom in unlocked medicine cabinet, shampoos and conditioners inside the tub, and bath salts and body sprays on an open shelf in the bathroom where accessible to children in care. Type B citation issued. See LIC 809D. LPA observed infant child placed in prohibited item (walker) in the daycare in bedroom #2. Type B Citation issued. See LIC 809D. LPA observed in bedroom #3 loose items such as blankets placed inside of the crib and play yards. Type B Citation issued. See LIC 809 D. LPA observed a mobile attached to the side of a play yard in bedroom #3. Type B citation issued. See LIC 809D. LPA observed no infant sleep charts for enrolled infants up to 24 months of age in care. Type B citation issued. See LIC 809D. At approximately 2:00pm, LPA observed licensee place infant under 12 months to sleep on stomach. Infant does not have a LIC 9227 completed on file. LPA reviewed 4 children’s files and observed them to be missing forms: LIC 282, and LIC 995A, and immunization records. Type B citation cited. See LIC 809D LPA provided copies of the forms for completion by parents of children enrolled in the facility. LPA observed LIC 9227 missing for infants enrolled up to 12 months of age in care. LPA Richmond observed the licensees updated mandated reporter training completed on 12/09/23 and compliance with immunizations requirements for childcare providers. Licensee has a current 1st aid/cpr completed on 10/15/23.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep web page at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. During file review LPA observed missing infant sleep charts. Type B citation cited. LPA provided licensee with infant sleep charts for daily completion.


Incidental Medical Services (IMS) are not currently being provided.

Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA Richmond reminded Licensee of the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of 100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&R) throughout California.

LPA provided applicant with the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. LPA Richmond gave applicant a packet of all required forms, and posters and reviewed following forms:

In addition, LPA Richmond informed applicant of the following:

o There is an effective 24/7 ban on smoking tobacco in a home that is licensed as a family day care home, and in those areas of the family day care home where children are present.
o Children and Staff records must be maintained and updated as needed and must be available for review by the Department.
o Saucer chairs, bouncers, walkers, or any similar items are prohibited.
o All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
o LPA provided and advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov

Deficiencies were cited today, per Title 22 Regulations and Health and Safety Codes. See LIC 809D.

An exit interview was conducted in Korean by interpreter ID # 15727 with licensee Byeongim Park . A copy of report and appeal rights were provided to licensee.

Notice of Site Visit provided and required to be posted for 30 days.
