32 | LPA Carus was informed prior to their visit that the school has already instituted a policy/plan of action to prevent an incident of this nature in the future.
Additionally, information revealed that the center director reported this incident to CDSS within the timeframe required, and was advised to report all incidents that occur to the parent or representative of the child or children affected as soon as they have a full understanding of the entirety of the incident that took place, and preferably on the date that they occur if possible.
Based on the evidence & information obtained during the investigation, which included interviews with relevant parties, & relevant documentation the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated.
1 Type-A Violation will be cited today.
LPA Carus informed director, Melanie Hepner that this report dated 4/29/2026 documents 1 Type-A citation which shall be posted for 30 consecutive days as there is/was an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Carus informed the director to provide a copy of this licensing report dated 4/29/2026 that documents any Type-A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
