25 | Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced annual required inspection to the above facility on 05/07/2024. LPA arrived at the facility at 12:00 PM, identified self and met with Maria Banalian, Licensee, who guided analyst on a tour of the inside and outside of the facility. There were 13 children present at arrival. Also present was Staff #1 (S1). LPA provided Licensee with a copy of the LIC125 Entrance Checklist to help facilitate the inspection. Facility operation hours are Monday to Friday from 08:30 AM to 5:30 PM.
The family child care home is a one-story studio ADU dwelling, includes combined/ living /dining /bedroom /kitchen and 1 bathroom. LPA toured the home inside and outside and a census was taken. Current facility sketch reviewed, and Licensee confirmed that combined living/dining/bedroom/kitchen and bathroom (located near entrance) are used for providing care and are accessible to children. The indoor combined living/dining/bedroom area is used for child care activities, eating, and sleeping, the area is set up with TV (mounted to the wall) children’s tables, children’s chairs, play items, toys, books, beds and bedding. The outdoor area includes a fenced play area that is located behind the second home on the property. The children use the bathroom near the entrance. Per Licensee, there are no indoor areas off limits to children and parents. Licensee stated after child care hours, use of the area for sleeping by the use of an air mattress and bedding, which is kept in the closet during hours of care. The licensee provides food for children in care. The kitchen is off-limits and made inaccessible by use of child proof locks. There is telephone service via a cellphone that is used, and the cellphone stays at the facility during operation hours.
Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The licensee obtained a signed Property Owner/Landlord Consent form (LIC 9149).
