Licensing Program Analyst (LPA) Elicia Calvillo and Licensing Program Manager (LPM) Rita Ramos conducted an unannounced annual continuation visit on 06/18/2025. LPA and LPM met with Lena Asatryan, Director, who provided a tour of the facility. LPA and LPM observed 19 children with 3 staff upon arrival.

The purpose of the visit is to document citations for deficiencies observed on 05/30/2025 during the annual inspection visit.

On 05/30/2025, LPA and LPM observed the facility to be overcapacity which is an immediate health, safety, and personal rights risk to children in care. LPA and LPM observed a total of 42 children at the child care facility. On 05/30/2025, LPA and LPM observed that Lena Asatryan’s ex-husband, Ara Asatryan, from the Asatryan Family Child Care Home #197493615, was present at the Child Care Center with 16 children from his child care home. Commingling children from one licensed Family Child Care Home with children from the licensed Child Care Center is an immediate health, safety, and personal rights risk to children in care. Per Director, Lena Asatryan, they were not overcapacity by 16 but rather 11 children. LPA and LPM informed Lena Asatryan, that even if they disagree with the census count of 16 that LPA and LPM counted, the facility was still overcapacity on 05/30/2025 since the facility is licensed for 26 and the census of 26 should not be exceeded.

During the 05/30/2025 visit, LPA and LPM were consistently informed by Lena Asatryan that she is the Licensee of the facility. Lena Asatryan informed LPM and LPA that they are present at the Child Care Center as both the Director and the Licensee from 7:00AM to 7:00PM. LPM and LPA documented on the 05/30/2025 visit report as Lena Asatryan being the Licensee.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

On 05/30/202025, Lena Asatryan, spoke to LPA and LPM in English and later was accompanied by their son, Sergey Asatryan, in who Lena Asatryan insisted that their son answer any questions for LPM and LPA on their behalf. LPM and LPA advised to speak to Lena Asatryan directly since Sergey Asatryan, is not listed as a Director, Assistant Director, teacher, or administrator and had a stern and hostile demeanor towards LPM and LPA. Although Lena Asatryan spoke in English, LPM and LPA also advised Lena Asatryan that an interpreter can be contacted. LPM contacted LPA Joe Katrdzhyan, in an attempt to speak with Lena Asatryan in Armenian, however, Sergey Asatryan stated and informed their mother, Lena Asatryan, that an interpreter was not necessary. Lena Asatryan stated that as a Licensee, they are giving their son Sergey Asatryan permission to speak to LPM and LPA on her behalf regardless of the requests and advisories LPA and LPM made.

Lena Asatryan made several statements that they expect to have their application for an increase approved before children return to school from their summer break. Lena Asatryan also stated that they know for a fact that everything they themselves submitted to the department is accurate and will indeed get her capacity increase. LPM kept informing Lena Asatryan that they are out of compliance and there is no guarantee about the timeframe to get licensed for their capacity increase.

LPM and LPA questioned Lena Asatryan about the presence of Ara Asatryan at the Child Care Center. Due to LPM asking if Ara Asatryan is Lena Asatryan’s husband or ex-husband, both Sergey Asatryan and Lena Asatryan replied with discontent and in a negative tone as to why LPM was using the term “husband” when Lena Asatryan has previously referred to Ara Asatryan as their ex-husband and has been divorced from Ara Asatryan for over a decade.

On 05/30/2025 LPM and LPA documented on the inspection report observing some discrepancies on some of the documentation obtained. LPA and LPM informed Lena Asatryan and Sergey Asatryan that a thorough review of Licensee's facility file will take place to ensure that there are no other errors or concerns regarding the capacity increase application, Licensee, and documentation obtained. LPM and LPA also documented that the Certificate of Occupancy provided that is dated 08/07/2024 reads that the maximum number of occupants is 36. LPM and LPA informed Lena Asatryan on 05/30/2025 that the number of occupants refers to the maximum number of people that can be in the entire building and it is not the same as the licensed capacity of 26 nor the measured activity space, or the requested number of 36 children Lena Asatryan is applying for.

In review of the facility file, LPM and LPA have found that Lena Asatryan is not the Licensee for Little Wonderland LA #197494942. Licensee is Ara Asatryan for Little Wonderland LLC. In communication with the Los Angeles Department of Building & Safety, LPM and LPA found that the Certificate of Occupancy is indeed the number of people/persons that can be in the building and not the number of children that they can care for at the facility. In communication with the Fire Department and the Inspector, the contact information they were provided with is for Lena Asatryan and not the licensee. Per the Fire Inspector, upon the Department’s request, the Fire Clearance will be adjusted to reflect the licensed capacity of 26 since they also confirmed that the Certificate of Occupancy reads a total of 36 occupants. As LPM and LPA informed Lena Asatryan on 05/30/2025, and as documented during today’s visit of 06/18/ 2025, the Department determines capacity based on the lessor number.

Further review of the file found that Lena Asatryan signed a Declaration Form on 10/21/2021 Under the Penalty of Perjury noting that they will not be at Asatryan Family Child Care Home #197493615 due to being the Director of the preschool. However, it is documented during an annual inspection dated 12/10/2024 that Lena Asatryan was present and met with the LPA Luque Avila who conducted the inspection. Lena Asatryan also sent emails as the facility representative for Asatryan FCC #197493615 inquiring on how to clear deficiencies for Asatryan FCC #197493615.

The Licensee for this child care center (Little Wonderland LA #197494942) is Ara Asatryan. The Licensee, Ara Asatryan, was licensed for Little Wonderland LA facility number #197494942, under the agreement and Declaration Form signed and dated 12/21/2021 that they will not be at the child care center (Little Wonderland LA #197494942) due to being the Licensee for Asatryan FCC # 197493615. It operates from 07:00AM through 10:00PM (as noted on LIC809 dated 12/10/2024 and he will be caring for the children enrolled in Asatryan FCC #197493615. Licensee for Asatryan FCC did not comply with their signed declaration since they were present with the children in their care at the Child Care Center on 05/30/2025. Although Lena Asatryan stated that the reason they were over capacity with the children from Asatryan FCC #197493615 present was due to a special event, neither Lena Asatryan nor Ara Asatryan informed the Department of their intention to have such event. The event caused the facility to be overcapacity and exceeded the number of maximum occupants as noted on their Certificate of Occupancy from the Los Angeles Department of Building and Safety.

During today's visit, the facility is made up of three (3) classrooms; Yellow Classroom, no children or staff present, Red Classroom, ages 4-5 years with 2 staff, and Green Classroom, ages 3-4 with 1 staff.

In completion of the annual inspection that took place on 05/30/2025, the following was observed at the facility.

When LPA and LPM observed the required posting in the entry way of the facility PUB269 Child Passenger Restraint System Poster was not posted.

During the inspection, LPA and LPM observed in the classrooms that the furniture, toys, and other equipment inspected were age appropriate and in good repair. There were individual cubbies available for children to store their personal items. Drinking water was available from filtered water and individual sippy cups labeled with each child’s name. Each child has access to a cot and bedding to nap. The restrooms were inspected and were stocked with supplies, sanitary, and in good repair.

LPA and LPM observed that children were being moved into and out of classrooms. LPA requested Sign in and sign out sheets from Lena Asatryan and were not readily available upon request.

There is telephone service, heating, lighting and ventilation were evaluated for safety and comfort.

LPA observed a first aid kits located in each classroom and inventoried for necessary supplies. LPA observed a device in each classroom, per FR it is a carbon monoxide detector. LPA observed smoke detector via fire alarm. Disaster drill log was not available, and Lena Asatryan was not able to provide the date of the last disaster drill conducted. Per FR, isolation area for sick children is in the office. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed cleaning solutions stored on a high shelf.

LPA did observe the kitchen area/ food preparation area, per FR meals and snacks are provided by the facility.

LPA and LPM toured the outdoor play area and observed that the outdoor play area was set up with two (2) portable merry go rounds and were monitored by two (2) third party vendor employees as identified by Lena Asatryan. The other playground equipment was in safe condition, free of sharp, loose, or pointed parts, in good repair, and appropriate for the ages of the children. During the tour, LPA and LPM observed along the back wall

of the play yard there was animal feces in the dirt area which was next to play equipment that children have access (photos taken). There is adequate shade in the play yard. The children have access to drinking water during outdoor play time. LPA and LPM did not observe a swimming pool or other bodies of water on the premises. During the tour of the play yard, LPM Ramos noticed a male individual leaving the play yard and entering the facility kitchen. LPA Ramos asked Lena Asatryan who the individual was and Lena Asatryan identifed the individual as Ara Astryan. LPA Ramos requested identification from the individual and confirmed it was Ara Asatryan. Ara Asatryan never stated that he was the Licensee for the facility.

LPA conducted a review of the staff and children’s records and found that they were incomplete. LPA requested a copy of the Personnel Roster, and it was not readily available upon request. LPA observed, staff records were missing transcripts to verify the teacher qualifications, proof of immunization against measles, pertussis, and influenza, proof of tuberculosis clearance, and Mandated Reporter Training Certificates. LPA requested a copy of the Child Care Roster, and it was not readily available upon request. LPA observed, children’s records were missing Admission Agreements and LIC995 Notification of Parent’s Rights.

Lena Asatryan was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Lena Asatryan and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed Liena Asatryan of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

Lena Asatryan was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During today’s visit, per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there is one (1) Type A Citation issued for Section 101216.3(a) Limitations on Capacity and Ambulatory Status and five (5) Type B Citations issued for Sections 101217(d) Personnel Records, 101229.1(d) Sign In and Sign Out, 101212(b) Reporting Requirements, 101238.2(d)(2) Outdoor Activity Space, and 101221(a) Child’s Records.

A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00.
