25 | Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced case management inspection on 06/17/2026. LPA arrived at the facility at 2:31 PM and met with Tara Perera, Director, who accompanied LPA on a tour of the facility. Upon arrival, 36 preschool children in care and supervised by 3 staff members.
Director Tara Perera left the facility. The remainder of the inspection was conducted with Assistant Director Annel Chavez.
LPA advised the Director that the purpose of the inspection was to monitor the facility's compliance with the Stipulation and Waiver and Order issued by the Department in January 2026.
During the inspection, LPA reviewed and discussed the requirements outlined in the Stipulation and Waiver and Order, including but not limited to: findings, revocation stayed with probation, strict compliance, site visits, fingerprint clearance requirements, personnel records, storage of toxins, posting requirements, children's rosters, staff hiring and training, incident reporting, building and grounds requirements, notice to parents, personnel requirements, and enhanced civil penalties.
LPA also reviewed the facility's obligation to post the Stipulation and Waiver and Order. During a previous inspection conducted on 01/27/2026, LPA discussed the terms of the order with Director Tara Perera and incorrectly advised that the order was not required to be posted. During today's inspection, LPA clarified that the Stipulation and Waiver and Order must remain posted in conspicuous location at the facility for the duration of the three year probation period.
