On 2/17/2022 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to Motivating Minds (Infant center), for the purpose of conducting a pre-licensing inspection. LPA met with Latiesha Austin (applicant) who provided a tour of the facility. The applicant is requesting a license with a capacity of 12 infants ages 18 – 23 months. The applicant also has a pending pre-school and school-age application 197494982/197494983 (combination center) that are located in the same building. The facility is a single-story building with 7 classrooms dedicated to day care, the infant room is located west of the reception area through restroom area. Operation days and hours are Monday thru Friday; 6:00 A.M. - 6:00 P.M.. There is an approved fire clearance on file conducted on 2/9/2022 by inspector Michael Judkins of the L.A. County Fire Department Prevention Bureau

LPA OBSERVED THE FOLLOWING OF THE INDOOR ACTIVITY SPACE

2AB10C or above fire extinguisher, smoke and carbon monoxide detector were observed inside classrooms and throughout the building.

First aid kits were available in all classrooms with the required essentials; scissors, bandages, tweezers, ointments and thermometer

Heating and Cooling was provided throughout the building using a central heating and cooling system.

The classrooms were clean in good repair, windows were in good repair free of chipping paint, dirt, insects or debris. Adequate lighting was observed

Trash cans used for solid or soiled did not have tight fitting lids

Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children, placed in a storage cabinet, applicant shall add locking device to doors

The reception area and general restroom will be used for isolation purposes, a cot shall be made available for resting .

The nearest working telephones were observed in the reception area, kitchen and directors office.

Center has cameras in all classrooms and surrounding outdoor perimeters.

Parents and authorized adults will use their original signature when signing children in and out. Sign in/out sheets shall be retained for 3 months or longer.

A parent board was observed in a prominent area, LPA provided the LIC. 311A for required postings updates

Age appropriate toys and equipment that promotes tactile stimulation, auditory, visual and manipulative skills were observed in good repair.

Storage for children’s belongings, diapers and other diapering needs were observed. Changing tables were within arm’s reach of a sink.

LPA observed cots in good repair for napping.The sleeping area was located inside the classroom. No swings or baby walkers were observed during today’s inspection..

Measurements for the indoor activity space was 260.40 divided by 35 SQ. FT. per child = 7 infants.

FOOD SERVICES

Parents shall provide prepared formulas in labeled bottles; all other food shall be labeled and properly stored.


High chairs with wide bases were observed for feeding. All classrooms were equipped with refrigeration to store breast milk and other foods capable of spoilage. The director shall be advised that in addition to formulas and foods provided by the parents, the center shall have an emergency supply of these items. Feedings shall be according to child's needs and service plan.

Center has devised an Incidental Medical Service plan to provide to parents of children with allergies (epi-pen), asthmatic (inhalers), and children needing G-tube feeding

RESTROOMS

There was one changing table located inside the classroom within arm’s reach of a sink. Children being potty trained will use the common restroom and a potty chair ring will be used to assist children when using toilets. Age appropriate toilets were observed. Cubbies were observed in this are for infant’s toileting supplies.

OUTDOOR ACTIVITY SPACE


The outdoor area was partially enclosed, repairs shall be made to completely enclose yard.

Age appropriate equipment was not observed during todays visit

Resilient cushioning and asphalt turf was observed in the play yard,

Water will be provided using portable water pictures and paper cups

Shading and benches for resting where not observed

Measurements for the outdoor activity area was 1252 divided by 75 sq. ft. per child for capacity total of 16 children utilizing the outdoor activity area at one given time.

Based on today’s inspection the facility shall be recommended for a capacity of 7 infants determined by the indoor activity measurements.

Applicant was made aware that state law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.



LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

Applicant was reminded that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and to never shake a baby to prevent the Shaken Baby Syndrome.



LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was also reminded that only children eating may be in highchairs and that car seats are utilized only for transportation.

The "Notification of Parent's Rights" (PUB394) was discussed with the licensee and the licensee was advised that it must be posted in a prominent area accessible to parents and visitors. Applicant was made aware that parents must sign and be provided a copy of the LIC. 995e

Criminal Record Statement

Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Applicant was provided the link to register with Guardian (guardian@ca.gov).


The following recommendations were requested prior to licensure.
