On 11/12/2024 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of delivering findings for the above-mentioned allegations. LPA was greeted by Lead Teacher V. Tamayo and discussed the purpose of the visit. LPA toured the facility and observed six (6) children in care with one (1) staff member providing care and supervision. Shortly after at approximately 10:00am Director Dana Davis arrived at the facility. Present during today's inspection were Lead Teacher V. Tamayo and Director Dana Davis.

On 08/22/2024 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations. Upon arrival, LPA met with Lead Teacher V. Tamayo and discussed the purpose of the visit. LPA toured the facility and observed 9 children in care with one staff providing care and supervision. Shortly after Director Dana Davis arrived at the facility at approximately 10:05 a.m.

During the course of the investigation LPA requested and reviewed the following documents: Children's roster, staff roster, children’s files, personnel records, sign-in/sign-out sheets from July and August, and staff timesheets for July and August.

Based on LPA observations, interviews of all relevant parties and record review, no information was disclosed that children’s personal rights were violated and/or that the facility is not providing adequate supervision to children in care and/or that the facility operates out of ratio. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegations are UNSUBSTANTIATED.



An exit interview was conducted, and report reviewed with Director Dana Davis. A copy of this report and
appeal rights were discussed and left with Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
