On 04/16/2025 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of delivering complaint findings. Upon arrival, LPA was greeted by Director Dana Davis and discussed the purpose of the visit. LPA toured the facility and observed fourteen (14) children in care with one staff member (S3) and Director Davis providing care and supervision. Shortly after at approximately 1:00pm Lead Teacher V. Tamayo arrived at the facility. Present during today's inspection were Director Davis and two (2) staff members.

On 01/22/2025 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations.

During the course of the investigation LPA requested and reviewed the following documents: Children's roster, staff roster, children’s files, and children’s diapering information. On 04/03/2025 LPA concluded interviews of all relevant parties.

Based on LPA observations, interviews of all relevant parties and record review, no information revealed that staff left day care child soiled for extended periods of time, staff did not provide adequate supervision to day care child, staff left day care child in highchair for extended periods of time, staff did not meet day care child's needs, staff did not ensure facility was in ratio and staff did not allow responsible party to communicate with other staff. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted with Director Davis. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
