On 9/11/2023, Licensing Program Analyst (LPA) Dalicia Adkins conducted an announced case management inspection visit. LPA Adkins met with licensee, Amalya Sedrakyan and licensee husband who guided LPA Adkins on a tour of the facility. Five children and one child care assistant present during inspection. Operation days and hours Monday- Friday 8:30am- 5:00pm. Licensee is Armenian speaking and husband assisted with translation.

The purpose of today’s inspection is to inspect home for an increase of capacity. Licensee currently operate a small family child care with 8 children and is requesting to increase to large family child care home to care for up to 14 children. Fire Clearance granted for up to 14 children, dated 05/04/2022. LPA inspected the entire facility including outside play area.
This is a single family, single story (no stairs) four bedroom home with two bathrooms (children utilize bathroom in the hallway) , one kitchen(off limits) , living room (used for primary care) , dinning room (off limits) laundry room (off limits) and attached garage (off limits) Off limits rooms identified on facility sketch and made inaccessible by locked door and child security gates.

Parent will use utilize street parking in front of the home and driveway. Parents will escort children to the front door, licensee will greet and receive children at the front door. Children cubbies are located near the front door entrance. Children have individual cubbies for personal belongings. At the front door entrance are double door with a ring and locked door. Parent Board posted near the front door entrance. LPA observed required posting: facility license (LIC 203), Emergency disaster plan (LIC 610A) Earthquake Preparedness Checklist (LIC 9148) Notification of Parents right (PUB 394).

LPA reviewed children and licensee files. Licensee has current Pediatric CPR -Pediatric First Aid certificates, expires on 01/02/2025 and mandated reporting completed on 02/07/2022. LPA Adkins observed First Aid kit on the top of the refrigerator.
All adults who reside or work in the home have a criminal record clearance. There are no excluded individuals present at this home. Day care will be conducted in the living room located to the left after entering the home. Children will also use this room for eating.
LPA observed safe and age appropriate toys, play equipment and materials.

There are two bathrooms in the home. Bathroom #2 is located next to the master bedroom and is off limits. Children will utilize bathroom #1 located to the right down the hall. LPA observed one operable toilet and two sinks. Bathroom is clean, safe and free of hazards. LPA observed one stepping stools and hygiene supplies available for use.

Children will nap in bedroom #1 located to the right. LPA observed age appropriate beds and in good condition. LPA observed one pack and play located in the living room area. There is currently one infant in care. No baby walker shall be allowed on the premises of the family childcare home in accordance with Health and Safety Code. LPA discussed designating a area in the effect of a child who becomes ill while in the home. Ill children shall be separated from other day care children and licensee shall maintain visual supervision of all children. LPA discussed safe sleep regulation with licensee. LPA reviewed LIC 9277 and pin 20-24 approved safe sleep regulations and copy given.

The kitchen is located next to the living room and is off limits. The kitchen is made inaccessible by children safety gates. Licensee will provide meals; breakfast, lunch and snacks. Licensee will also provide filtered drinking water. LPA observed water filer system located in kitchen. Water shall be ready and available at all times for children in care. Sharp utensils made inaccessible. LPA observed operable refrigerator and is clean.

Per Licensee there are no weapons or firearms on premises. LPA Adkins observed the home to be ventilated, working heating system and working cell phone (818) 300-3434. Cleaning compounds, medicines and sharps are inaccessible to children. Fire extinguisher located in room kitchen , serviced on June 24,2023. LPA observed three carbon monoxide detector; one mounted on the kitchen ceiling, living room ceiling and bedroom #1, where children naps. LPA observed smoke detector alarm system located in the kitchen. LPA observed fire place and is made inaccessible by screened glass. No smoking in the home and is prohibited. Per licensee there are no firearms in the home. All cleaning compounds were made inaccessible and medication made inaccessible. Currently licensee is not providing incidental medical services. There are currently no pets in the home.

Children will use the front yard for outdoor playtime, front yard is fenced. LPA observed age appropriate toys and are in good condition. Play yard is free of debris and hazardous materials. The front yard has a grass area and shaded area.

Back yard is off limits. In the kitchen there is a sliding door that leads out to the back yard area. The sliding glass door leads to the patio area and pool is next to the patio. The pool is enclosed with a wrought iron gate with a lock. A key code in needed to open the lock. The gate door swings away from the pools and is self latching. The fence is at least 5 feet high. Per licensee childcare children will not use the pool and will remained locked during childcare hours. There are two other entrances to the back yard area. There is another sliding glass door in the master bedroom that leads to the backyard. There is a lock with an alarm on the sliding door glass. Licensee demonstrated how the alarm works, LPA observed alarm triggered when door opened. LPA took short video. The third access to the backyard area is from the living room. The living room is off limits. There is also an alarm on this sliding door and window that opens up to the backyard. In addition to the alarms there is also professionally installed window shutters that can be closed.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/childqanda.htm Child Care Licensing Program has strengthen regulations around safe sleep. The new requirements are consistent with the American Academy of Pediatrics safe sleep guidelines, and CCLP seeks to
connect licensed care facilities with the regulations and resources to improve environments that could otherwise put infants at undue risk. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep website at
http://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www.cpsc.gov/ and recommended they register all infants devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA and Licensee discussed the Community Care Licensing website
www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home.

A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. There were no proof of corrections discussed during this visit. Manager final approval will be required prior to being granted a increase in capacity.

A notice of site visit was given and must remain posted for 30 days.
