Facility is located on the property of Well Spring Church. Facility is accessible south of the Sanctuary. LPA Lowe toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 2 classrooms designated for infants and toddlers.. Children will have access to 2 sinks and 4 potty training seats.

Classroom measurements are:

Indoor total measurements: 939.65 square (sq) feet (ft) divided by 35 sq ft = 26 infants.

Application requested capacity is 23.

Outdoor Play measurements are:

Outdoor total measurements: 2266.13 sq ft divided by 75 sq ft = 30 infants/toddlers.

Facility has two outdoor play areas, one designated for infants and one designated for toddlers.

Indoor/Children’s Area:

Indoor activity space for infants was observed to be physically separate from space used by children in the toddler option and children in the child care center.

Toys were observed to be safe, with no sharp points or edges, or be made of small parts that can be pulled off and swallowed. LPA advised rattles shall be large enough so that they cannot become lodged in an infant's throat and constructed so that they will not separate into small pieces.

LPA observed individual permanent and portable storage space for children’s personal belongings. All materials and surfaces accessible to children, including toys, was observed to be free of toxic substances.

LPA observed a water cooler available for drinking water. Facility has a monthly pest control service to assist in keeping the center free of flies, other insects, and rodents. All trash cans were observed with tight fitting lids in good repair. No Fireplaces or open-faced heaters were observed at facility.

LPA advised disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. Storage areas for poisons shall be locked. LPA advised shall maintain first-aid supplies in a location accessible to staff but inaccessible to children. LPA observed a current edition of a first-aid manual, sterile first-aid dressings, bandages or roller bandages, adhesive tape, scissors, thermometer, and antiseptic solution.

Facility isolation area was observed to be in the director office. LPA advised any infant in the isolation area shall be under constant visual observation by a director, an assistant director, a teacher or an aide.

The isolation area has a cot for each ill infant. LPA advised a crib or pack play can be used in the isolation area for infants in addition to the cot. LPA advised the isolation area shall not be located in the kitchen area or the general-use toilet area.

Director stated facility will use written to sign the child in/out of the child care center. LPA advised the licensee shall be responsible for ensuring that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. Director stated wellness checks will be conducted every morning at drop off.

LPA advised smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). LPA advised a baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c).

LPA advised there shall be at least 35 square feet of indoor activity space per child based on the total licensed capacity. LPA Lowe observed 2 sinks and 4 potty chairs. LPA advised there shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every five infants being toilet trained.

Napping:

LPA observed 8 cribs to be used for each infant who is unable to climb out of a crib: LPA advised cribs shall not limit the ability of staff to see the infant. Crib mattresses were observed to be new covered with vinyl and observed in good condition with no exposed foam.

Cots were observed for all infants who have the ability to climb out of a crib. LPA advised each crib or cot shall be occupied by only one infant at a time. Each infant's bedding shall be used for him/her only. LPA advised such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant.

The calculation of indoor activity space for infants was not included in space designated and used for cribs. The classroom was observed with a 1/2 wall to ensure infants crib space is physically separate from the indoor activity space.

Cots used for napping were observed to be new and in a safe condition. Covered with vinyl that can be wiped. Director stated cribs and cots will be wiped with a detergent/disinfectant weekly or when soiled or wet.

Facility was observed to have a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.

Infant Changing Tables

LPA observed 4 changing tables to have a padded surface no less than one-inch thick and be covered with washable vinyl or plastic. Observed with raised sides at least three inches high and maintained in safe condition. LPA advised while in use, be placed within arm's reach of a sink. Sinks in classrooms are portable. Not be located in the kitchen/food-preparation area.

Outdoor:

Outdoor activity space was observed to be physically separate from space used by children in the child care center. LPA observed a shaded rest area for the children. Outdoor area was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. Grass area was observed with a little tykes slide and a larger little tykes climbing equipment. The larger climbing equipment was observed with cracks and broken pieces. Applicant stated broken equipment will be removed from outdoor area. If the facility adds additional equipment LPA advised the areas around and under high climbing equipment, slides and other similar equipment shall be cushioned with material that absorbs falls.

The playground was observed to be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence was observed to be at least four feet high.

LPA advised any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. This shall include, but not be limited to, incinerators, air- conditioning equipment, water heaters, and fuse boxes.

There were no pools or other bodies of water observed on the property.

Health Related Services:

LPA advised medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration. Director stated a locked box will be used for any medications.

Facility has a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Food Service:

LPA advised prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. LPA reviewed centers prepared needs and service plan. LPA advised high chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding.

LPA advised bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. The infant care center shall not serve honey or corn syrup to any infant.

LPA advised all food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner.

LPA advised all foods or beverages capable of supporting the rapid and progressive growth of microorganisms that can cause food infections or food intoxications shall be stored in covered containers at 45 degrees F (7.2 degrees C) or less.

LPA advised pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.

LPA advised food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin. Facility has a monthly pest control service.

Facility was observed with a food preparation area in a room located through the infant room with a door to make the room inaccessible to children.

LPA advised the food-preparation area shall not be used for: Children's play activities, unless such activities are part of a supervised food-education program.

Director stated infants will bring all food and bottles from home. Children who can eat table foods will be provided with two snacks a day and will bring their own lunch from home.

Postings:

LPA advised the license shall be posted in a prominent, publicly accessible location in the center.

LPA Lowe observed the following postings located in each classroom. LIC 613A (9/96), the PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems, and a contingency plan for action during fires, floods and earthquakes.

Menus were observed to be in writing and posted in each classroom to be review by the child's authorized representative. LPA advised copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

LPA observed facility has a disaster and mass casualty plan of action in writing.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

The following corrections are needed before licensure will be considered. Corrections are due by 7/15/2022:

· Remove large little tykes play equipment that is broken from the toddler outdoor play area.

An exit interview was conducted with the above items discussed and a copy of this report was provided to the Applicant/owner, Noesh Hordagoda and Gladys Villalobos, Director. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
