On 8/21/26, Licensing Program Analyst (LPA) Angela Luz conducted an unannounced subsequent complaint visit to deliver findings. LPA met with Director Amanda Dimson and informed them of the reason for the visit. LPA toured the facility inside and outside and noted 6 toddler children supervised by 1 staff and 16 infant children supervised by 4 staff. The facility is in ratio compliance today.

On 7/1/26, the El Segundo Regional Office received the above mentioned complaint allegations.

On 7/7/26, LPA conducted initial complaint visit and conducted staff interviews and took pictures of classrooms. Copies of incident reporting procedure, child supervision procedure, cleaning and sanitation procedure and safety tips, daily/weekly classroom cleaning checklist, and name to face documentation were obtained. Child records were reviewed. Throughout the investigation, LPA conducted parent interviews.

Pertaining to the allegation - Staff did not ensure day care child did not have access to hazardous chemicals:

Based in staff interviews, there was an incident that occurred during a transition in a toddler classroom where a child (C1), climbed up and into a large water/sensory bin that had toys inside. A staff (S1) took the child out. S1 noticed discoloration on C1's clothing. Staff helped C1 wash their hands and change clothes. It was discovered that the toys in the water/sensory bin were not fully dry after being cleaned the night prior. Some staff interviewed stated the toys in the sensory bin smelled like bleach. Bleach water is used at night to clean. Staff are trained to spray the bleach water solution on the materials, rinse them with water, and leave them to air dry overnight. Staff stated the water/sensory table was put in the teacher area of the room immediately after C1 was taken out of it. The clean toys were left inside.

On 7/7/26 LPA observed the sensory table in the teacher area of the toddler classroom.
LPA noted it was haphazardly filled with plastic, foam, and wooden toys. Based on the way the toys were arranged, there would be limited air flow between toys. LPA noted on 7/7/26 and 8/21/26 that cleaning supplies and cleaning solutions were stored out of reach of children.

Based on LPAs observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

Pertaining to the allegation - Staff did not provide responsible party with incident reports.

Based on staff interviews, staff write incident reports for physical injuries that resulted or could result in a mark on the child such as a scrape, bump, or bruise. Staff would write 2 incident reports for incidents that involve 2 children such as a bite. They would write one for the child who got bit and the child who bit. Staff stated they do not write incident reports for accidents or illnesses. 

Based on interview and records reviewed, staff did not write an incident report for the incident described above, regarding a child's clothing getting discolored due to bleach water solution. Staff informed the child's authorized representative of the incident verbally at pickup. Additional interview revealed the incident was not revealed until the child's authorized representative asked why the child was wearing different clothes than they were dropped off in.

Based on records reviewed, a training document titled "Incident Reporting Guidelines" states that "Any accident, injury, or illness involving children, regardless of severity, must be documented on an incident report and child injury and illness log…"

Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

2 Type B deficiencies are issued today. Appeal rights provided.
A notice of site visit was given and must remain posted for 30 days.

Pertaining to the allegation - Staff did not provide adequate supervision to day care child: The reporting party was concerned about the toddler program.

Based on parent interviews, there are no concerns with supervision provided in the toddler program. Several parents stated their child/ren mouth materials but they are not concerned with the supervision surrounding mouthing materials.

On 7/7/26 and 8/21/26, LPA noted that there was a maximum ratio 1 teacher to 6 toddler children in care, which is compliant with Community Care Regulations for teacher to child ratio in a toddler program. LPA observed on both dates that staff could see and hear the children in their care.

Based on the evidence obtained during the investigation, which included interviews with relevant parties, and information collected and reviewed from pertinent parties, there is insufficient evidence regarding the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the above alleged violations did or did not occur, therefore the allegation is found to be unsubstantiated.

No deficiencies are issued.
A notice of site visit was given and must remain posted for 30 days.
