On 4/10/2024 Licensing Program Analysts (LPA) Judy Laureano and Tyra Chavies arrived at above mentioned facility for the purpose of investigating the above-mentioned allegation. Upon arrival, LPAs met with Gabby Romero, Facility Director and discussed the purpose of the visit. LPAs toured the facility and observed 33 children in care with 8 staff and director providing care and supervision.

During the initial inspection at 12:40 p.m., LPAs observed Infant 1 room, which had 3 children who were awake being supervised by one staff member who was on the floor with them and one teacher doing diaper change for 1 infant. Additionally, LPAs observed 2 sleeping children in the nap room, with a short door and see-through window. LPAs did not observe the sleeping infants to have a staff supervising them visually and auditorily. LPAs requested to review the current sleep log of the two sleeping infants and director confirmed that the log was not completed. Based on
LPAs observation of staff seated on the floor and the lack of documentation required for the sleep log, facility is not providing adequate visual and auditory supervision, citation was issued, please see LIC 9099D.

LPA requested the following documents: children's roster, staff roster, name to face logs for January and February and parent handbook. Any documents not available during today’s inspection, director agrees to email LPAs.

Based on LPAs Laureano and Chavies’s observation of children in care, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

Exit interview conducted and report was reviewed with Director Gabby Romero.

A notice of site visit was given and must remain posted for 30 days.
