On 09/02/2025, Licensing Program Analyst (LPA) Adrian Risher made an announced visit to Hermosa View Star for the purpose of conducting a Case Management Licensee Initiated inspection. The applicant is requesting an increase to their school-age license from 200 to 300 children ages 4 -8 years of age. The program will be a before and after school program located on the Hermosa View elementary school campus. Per the Hermosa Beach City School District's approval of use of school facilities the following rooms will be dedicated to the school-age program, rooms: 225,303,401, 501, 502, 601, 703, 704 and the multipurpose room. The following rooms are being added to the school age program: 222, 301, 302, 303, 403, 603, 701, 702, 703, and 704. The agreement allows full use of the out door activity space. The agreement is from 08/20/2025 to 06/12/2026. The hours of operation are as follows: Monday -Friday 7:00 A.M. - 8:45 A.M./ Monday, Tuesday, Thursday and Fridays 2: P.M. - 6 P.M. and Wednesday 1:00 P.M.-6:00 P.M , the multipurpose room will be available Monday-Friday 7:00 a.m - 6: p.m. There is an approved fire clearance dated 7/7/2025 on file conducted by Matthew Alexander of the L.A.County Fire Department.

School was still in session when LPA arrived at the school. LPA observed 180 children in care once school was dismissed.

LPA observed age appropriate equipment and n good repair.

Drinking water is available both indoors and outdoors. LPA observed water jugs on each cart that the staff take with them to their classrooms and in the multipurpose room. Staff use disposable cups to provide water to the children. Filtered water faucets and bottled water will be available for an outdoor water source.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The school has central heating and cooling. LPA observed adequate lighting in the classrooms. The classrooms were clean and in good repair. Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children. The facility was equipped with working telephones inside the classrooms.

The program will provide an evening snack.

Staff shall sign children in and parents or authorized adults will sign children out. The required postings were also posted in a common area for parents and visitors review.

The center was reminded that an Incidental Medical Service plan shall be provided to parents of children with allergies (epi-pen), asthmatic (inhalers), and children needing G-tube feeding

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair.

LPA inspected the outside play area. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shade and benches for resting. LPA observed age appropriate toys and equipment were observed in good condition.



The following measurements were taken during today's inspection: Room 222 929.89= 26 children, Room 301 872.82= 24 children, Room 302 872.82=24 children, Room 303 872.2=24 children, Room 403 871.69=24 children, Room 603 1097.41=31 children, Room 701 1290.99=36 children, Room 702 1290.99=36 children, Room 703 1290.99= 36 children and Room 704 1290.99= 36 children. The total measurement for all of the classrooms measured during today's inspection is 10,681.08 which is a total capacity of 297.

Based on today’s inspection, the facility shall be recommended for a capacity of 300 school-age children. However, additional documents are needed regarding the occupancy numbers for the classrooms for final approval.

Per Health and Safety Code Sections the following exemptions shall apply:

1596.806(a), (a)(1), (a)(2) state: (a) A room used as a classroom by a school-age child care program shall not be required to meet the square footage or toilet requirements for child day care centers if the program is operated on either of the following:

1) A functioning school site in the same facilities that have housed school children during the day, before or after school hours, or before and after school hours.
(2) A functioning school site in facilities certified as usable as a classroom for instructions....

1596.806(b) (1) Fencing, outdoor activity space, toilet, and isolation space requirements....

Per Title 22 section 101538.3(d) The capacity per room for a school-age child care program that meets the requirements of Health and Safety Code Section 1596.806 shall not exceed the number of children for which a room of this size is commonly approved for school use during the school day.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Facility Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
