On 08/25/2025 Licensing Program Analyst (LPA) Cristina Castellanos conducted an announced inspection with applicants Shanice & Tyrone Richardson. The purpose of this inspection is to ensure the standards for a Family Child Care Home are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes.

The applicant is applying for a Large Family Child Care Home license with a max capacity of 14. The application is for an increase in capacity. Applicants have been licensed for a Small FCCH as of 10/28/2022. Control of property was submitted to the El Segundo Child Care Regional Office for review. Landlord notification is on file. On 08/11/2025 a fire safety inspection was completed, and a fire clearance was granted by the Los Angeles County Fire Department.

Per the application submitted, applicants wish to provide services to infants 6 weeks old to 13 years old 24 hours, Monday through Sunday. Applicants are available for overnight or weekend care. Applicant Richardson was informed that any changes to ages, hours and days of operation shall be submitted to the department via LIC 279 approval prior to initiation of changes.



The single-family home consists of a living room, a dining area, 3 bedrooms, 2 bathrooms, a kitchen, a laundry area, a family room, and a detached garage. Residing in the home are both applicants and their two minor children. Present during today’s inspection were both applicants, applicants two minor children and two children being cared for.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Applicant confirmed the following areas are OFF LIMITS to the children in care: all 3 bedrooms, bathroom number 1, the detached garage and the gated backyard (behind the family room). The kitchen and the laundry area will only be used as a walkway to the family room. LPA reminded applicants that any area designated as OFF LIMITS should be made inaccessible during the hours of operation and/or while children are present.

Families will enter through the main entrance door where they will be led to the main living room. Per applicant the family room will be the designated day care space. LPA observed a children size table with chairs, appropriate age toys, a mounted TV, a changing table, napping cots, a yourigami kids convertible play fort and a fisher price climbing Jungle Gym play set. The open-faced heater in the family room was observed to be barricaded.

LPA observed kitchen cabinets to have sliding cabinet locks making content inaccessible to the children in care. Knives and sharp objects are stored away above the refrigerator. LPA observed the stove with stove knob covers. Cleaning compounds are kept in the detached garage, making content inaccessible. Applicant confirmed she provides meals and snacks. LPA discussed the importance of having policies in place to ensure that all who help with care and supervision are aware of any dietary restrictions and/or allergies. LPA also encouraged applicants to contact their local Resource and Referral agency, Crystal Stairs, to inquire about any food programs and/or other professional development opportunities and resources.

Next to the family room is the children’s designated bathroom. Sink, toilet and bathtub were observed and inspected. The bathroom cabinet was observed with a safety sliding lock, making content inaccessible to the children in care.

The outdoor space that children will use was observed and inspected. Children will use the side yard of the home. LPA observed age-appropriate outdoor toys and a shaded area. A variety of outdoor toys were observed and inspected.

There are no firearms or ammunition on the premises. All electrical outlets in the home were observed covered. LPA observed a working fire extinguisher in the kitchen area as well as one in front of the family room. A working dual fire alarm & carbon monoxide detector was observed in the family room. Additionally, home has a pull fire alarm system.

LPA discussed Safe Sleep Regulations and resources with applicants. There is one crib or play yard for each infant in care, cribs and play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard. All cribs or play yards shall meet the United States Consumer Product Safety Commission safety standards. Infants are not swaddled while in care. Provider physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Infants can be visually observed through an open door if sleeping in a separate room. The Individual Infant Sleeping Plan will need to be completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping.

Applicants were reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicants the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to applicants.

Applicants were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During today’s inspection there were no corrections that needed to be made. The application determination for a capacity increase will be reviewed with Licensing Program Manager for final resolution.

An exit interview was conducted with applicants Shanice & Tyrone Richardson. A copy of this report was discussed and left with applicants. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
