25 | On 10/21/2025 Licensing Program Analyst (LPA) Doris Whitmore made an unannounced visit for the purpose of conducting a required 3-year inspection. LPA met with Site Supervisor Georgette Bradley and toured the facility indoors and outdoors. LPA observed proper care and supervision during this inspection. LPA observed a total of 22 children and 8 staff. Facility hours of operation are Monday- Friday 8:00a.m. to 4:00p.m. The day care is located on the YMCA campus, there is a separate entry from the public, located on 99th street side of the building, equipped with electronic entry and cameras.
LPA observed all postings requirements for operation on the postings board LIC203A- License, LIC610A-Emergency Disaster Plan, LIC9148- Earthquake Preparedness Checklist, PUB394-Notification of Parents Rights Poster, LIC613A- Personal Rights, PIB-269- Car Seat Law, Menus, Activity Schedule, Roster. The facility obtained current pediatric/ CPR and First Aid for all facility staff members. LPA Whitmore shared with the Site Supervisor the updated Personal Rights form. The updated Personal Form. was posted on the board. The phone number of the Health Department was also posted on the board.
During this inspection, LPA observed a working telephone on the premises, heating, lighting and ventilation were evaluated. Furniture and equipment were inspected for age appropriateness and good repair. Napping equipment was observed in good repair. Adequate storage for children's belongings, bathrooms facilities, and separate areas for isolation and care of ill children located in the office area. Documentation of current Fire/Earthquake disaster drill was observed. First Aid supplies were inventoried. First aid kits were available with the required essentials, scissors, bandages, tweezers, and thermometer Sign in/out were observed. A review of medication policy, including properly labeled and stored in original container. During today’s inspection, the food allergy list is posted in every classroom and kitchen. Page 1
