On 10/27/2022 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to Village Tree Infant Center for the purpose of conducting a pre-licensing (change in facility type) inspection, the center was previously a preschool owned by the same corporation. LPA met with Kimberly Kern – Managing Director and Cindy Renderos Contreras – representative and a tour of the center was provided. The applicant is requesting an Infant license with a capacity of 18 infants, ages 8 weeks - 24 months. The building is a converted 2 story home with a detached garage, infant care will be conducted on the lower level, the upper level is used as the directors and staff office (off limits with gated stair case), and the detached garage will be used as for mommy and me classes/isolation area (not included in capacity calculations).
There is an approved fire inspection on file conducted by Inspector Sivaborvorn of the L.A City Fire Department – Institution Unit.

The following was observed of the:

INDOOR ACTIVITY SPACE: Title 22, sect. 101438.3

Fire extinguishers were 2AB10C or larger was observed

Central heating and cooling systems are used to control classroom temperature.

Adequate lighting was observed. Windows screens were in good repair and free of insects, dirt and debris

The classrooms were clean, rugs and floors were in good condition

The center was equipped with cameras indoors and outdoors

LPA observed an adequate amount of storage bins for children’s belongings.

Trash cans used for solid waste were observed with tight fitting lids

LPA did not observe carbon monoxide detectors during todays inspection. During the visit carbon monoxide detectors were placed on order

First aid kits were observed with the required essentials: scissors, bandages, tweezers, antiseptic solution and thermometer

Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children, placed a storage rooms and locked cabinets.

The mommy and me/converted garage will be used for isolation of ill children. LPA advised that a crib or pack and play is needed for infant that are required to be in a crib and a potty chair are readily available. The room is equipped with a changing table. There is no running water, LPA suggests a portable sink be added.

The center has a working telephone service for communicating.

Parents and authorized adults will sign in using an electronic sign in device - "Child Pilot”. Applicant is advised that sign in sheets shall be made readily available for review by the department, for a minimum of 30 days.

The required postings shall be posted in a prominent area for parents and visitor's view.

LPA observed age appropriate toys and furniture. The combination of toys shall provide and encourage: Auditory stimulation, visual stimulation, tactile stimulation, manipulative skills

The napping areas were in within the classroom; 12 standard cribs, with no drop down sides were observed for napping. Four of the cribs were equipped with emergency wheels for evacuation. Separation is provided by a partitions 4 feet or higher.

Applicant is hereby advised to ensure form # LIC. 9227- Infant Sleeping Plan to the infants file folder be added to the infants file. All service plans shall be adjusted according to child's needs.

LPA observed age appropriate, low feeding chairs and tables. Applicant is advised infants that are unable to sit in chairs, shall be held and fed by staff members.

There was one changing table within arm reach of a sink located in each section of the activity areas.

No baby walkers or bouncers were observed during today inspection.

The measurements for the indoor activity space was 632.14 divided by 35 SQ. FT. per child = 18 children

FOOD SERVICE: Title 22, sect. 101427

Parents will provide lunches and the center will provide A.M and P.M. snacks for the older infants, parents are to prepare bottles, the center will heat them according to children’s service plan. It is recommended that bottle fed infants are fed at least every four hours. The center is hereby informed that a supply of formula and foods shall to be available for emergency purposes, emergency formula can be provided by parents or the childcare center.

The center has a food prepping kitchen with refrigeration and heating source for infant foods and formulas (formulas will be heated using the crock pot method), a refrigerator was observed for storing of formulas, a thermometer is needed. The prepping area was clean in good condition. LPA did not observe any contaminated foods or hazardous conditions in this area, this area is inaccessible to children in care. Infant bottles and foods shall be properly labeled with child’s names and date of preparation. LPA advises that the prepping area be made inaccessible to children in care. Per the managing director children in room 1 uses this area to get to the back-play yard. A plan to have children rerouted to gain access to the play yard shall be devised.

Applicant was informed that an individual feeding plan shall discussed and made with the authorized representative and/or physician, prior to the infants first day of care. This plan shall be updated as needed or at the authorized representative's request.

Weekly menus were posted for review. Applicant shall make preparation for alternate meals for children with allergies. The plan shall be in accordance with an Incidental Medical Service plan, and discussed with parents of children with, allergies (epi-pen); asthmatic (inhalers); and children needing Gastronomy tubes (feeding G-tube). Additional information was provided with this licensing report.

Applicant is hereby informed that regulations referencing infant care can be located in Title 22 Division 12 Chapter 1 Sub-chapter 02.- Infant Care Center

RESTROOMS: Title 22, sect. 101439(i)(1)(2)

THERE WERE:

2 toilets and 1 urinal = 1 toilet per 15 children with a urinal per every two toilets = 3 toilets for a total of 45 children.

5 sinks = 1 sink per 15 children for a total of 15 children for a total of 75 children. There was a changing table within arm reach of a sink, in each section of the infant center and 3 sinks in the restroom. Fixtures were age appropriate. LPA observed a stackable washer and dryer and an area for lactating mothers inside of the restroom. LPA advised that a locking devise be added to the dryer door, during the inspection the lock was added.

LPA observed a staff restroom in classroom #2 (with a shower), LPA advised that this restroom be made inaccessible to children in care and that slow jams be added to both restroom doors to prevent injury.

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water only.

LPA observed 3 separate play areas; the front yard, two sections in the back yard separated by gates.


A minimal amount of age appropriate toys and equipment were observed in the outdoor activity space in good repair. Per the manufactures label on the climbing apparatus recommended use for children ages 2 -5 years. Staff shall ensure that only children within the recommended age be allowed to make use of this equipment with adequate supervision. Per managing director, the yard will be used for non-walking infants. Artificial grass was observed for cushioning under the climbing apparatus and other play areas. LPA observed a locked sand box, sand box shall be inspected every day and kept free of hazardous foreign materials.
The play yard was fully gated with a gate 4 feet high or higher.

Shade sails were available for shade, and benches for resting

LPA observed a protruding water pipe in the play area on west side of the building, it shall be made inaccessible to children in care. Also the wooded bench shall be covered to prevent splintering. There needs to be an alerting mechanism on the emergency exit gates on the east side of the center. Center needs to devise a plan to make the concrete step leading into the mommy and me classroom, non-hazardous to children in care and a gate shall be added to the bottom of the steps leading to the play yard.



Measurements for the outdoor activity space = 1718.64 divided by 75 SQ. FT per child for a total of 22 children

Ratios and supervision were also discussed during todays inspection according to Title 22 sections 101416.5 and 101416.8

Based on measurements and observations the center shall be recommended for licensing for a total of 18 infants, determined by the applicants requested capacity.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
