On 12/21/2022 Licensing Program Analyst (LPA) made an announced visit to My First Place-Virginia Ave. infant center for the purpose of conducting a change of ownership, pre-licensing inspection. Upon arrival LPA met with Sean and Maria Kirshinstein - applicants. The applicants are requesting an infant license with a toddler option for 20 infants; ages 0-2 years of age and 20 toddlers ages 18 - 36 months. Applicants also has a pending application for a preschool component (197495210).
The center has a total of 5 classrooms, two classrooms located to the right of the main entry will be used for infant care. During todays visit there were a total of 6 infants, LPA observed proper care and supervision and ratio's were maintained. During today visit applicant requested an increase to the capacity from a combined total of 32 -40. Per inspector Chris Davis of the Santa Monica fire department he will provide an updated fire clearance. Days and Operation: M-F/ 7:30.a.m - 6:00 p.m.

The classrooms were clean, rugs and floors were in good condition
The center was equipped with cameras indoors and outdoors
LPA observed an adequate amount of storage bins for children’s belongings.
Trash cans used for solid waste were observed with tight fitting lids
LPA observed carbon monoxide detectors during todays inspection
First aid kits were observed with the required essentials: scissors, bandages, tweezers, antiseptic solution and thermometer
Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children, placed a storage rooms and locked cabinets.
The infant tummy time area will be used for ill infants, applicants is advised that there shall be no other children present during this time and there shall be constant supervision of the

child. Children will use one of the two staff restrooms shall they become ill; applicant shall provide equipment to assist the child when using the standard sized fixtures. The required postings were posted in a prominent area for parents and visitor's view.

The center has a working telephone service for communicating. Parents and authorized adults will sign in using an electronic application (Pro-Care). Applicant is reminded that sign in sheets shall be made readily available for review by the department, for a minimum of 30 days.

LPA observed age appropriate toys and furniture. There were toys provided and encouraged: Auditory stimulation, visual stimulation, tactile stimulation, manipulative skills

LPA observed a separate crib room with 8 cribs, toddlers shall nap on mats. Applicant is hereby advised to ensure form # LIC. 9227- Infant Sleeping Plan to the infants file folder be added to the infants file. All service plans shall be adjusted according to child's needs.

The infant care center shall obtain written permission from the child's authorized representative for the placement of the child in the toddler program.

LPA observed age appropriate, low feeding chairs and tables. Applicant is advised infants that are unable to sit in chairs or hold their bottle, shall be held during feeding, bottles shall not be propped during feedings.

There was one changing table within arm reach of a sink located in each section of the activity areas.


No baby walkers or bouncers were observed during today inspection.

The measurements for the indoor activity space was 1676.02 divided by 35 SQ. FT. per child = 47 children

FOOD SERVICE: Title 22, sect. 101427

The center will provide meals and snacks for the older infants, parents are to prepare bottles, the center will heat them according to children’s service plan. The center is hereby informed that a supply of formula and foods shall to be available for emergency purposes, emergency formula can be provided by parents or the childcare center.

The center has a kitchen with refrigeration and heating source for infant foods and formulas. LPA did not observe any contaminated foods or hazardous conditions in this area, this area is inaccessible to children in care. A refrigerator was observed in the classrooms for storing of formulas and foods; applicant was reminded that thermometers were needed. Infant bottles and foods shall be properly labeled with child’s names and date of preparation.

Applicant was informed that an individual feeding plans shall be discussed and made with the authorized representative and/or physician, prior to the infants first day of care. This plan shall be updated as needed or at the authorized representative's request.

Weekly menus were posted for review. Applicant shall make preparation for alternate meals for children with allergies. The plan shall be in accordance with an Incidental Medical Service plan, and discussed with parents of children with, allergies (epi-pen); asthmatic (inhalers); and children needing Gastronomy tubes (feeding G-tube). Additional information was provided with this licensing report.

THERE WERE:

2 toilets = 1 toilets per 15 children for a total of 30 children. Applicant shall provide a potty chair for


4 sinks = 1 sink per 15 children for a total of children for a total of 60 children.

Fixtures were age appropriate. The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water only.

LPA observed that there was 1 toilet and one sink in need of repair, per applicant informed.LPA that a plumber had been scheduled to repair the fixtures.

OUTDOOR ACTIVITY SPACE: 101438.2

The play yard was fully gated with a gate 4 feet high or higher.

Artificial grass was observed for cushioning under climbing equipment, toys and equipment were age appropriate and in good repair. Shade sails were available for shade, and benches for resting.



LPA observed a protruding water pipe in the play area. LPA did not observe any bodies of water in this area during todays visit.

Measurements for the outdoor activity space = 1692.32 divided by 75 SQ. FT per child for a total of 22 children. Applicant is requesting a waive to use shared outdoor space and to waive the 75 square footage requirements.

Based on measurements and observations the center shall be recommended for licensing for a total of 20 infants and 20 toddlers, determined by the applicants requested capacity.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
