On May 9, 2023, Licensing Program Analyst (LPA) Doris Whitmore conducted an announced inspection with applicant Latonya Carr at 215 E. Plymouth#1 Street Inglewood CA 9032. The purpose of this inspection is to ensure the standards for a Family Child Care Home are being met in accordance with California Title 22 Regulations and California Health and Safety Codes. LPA met with La Tonya Carr who guided a tour of the inside and outside of home. Currently there is no back yard. In the front of the building are designated areas for parking and garages.

The applicant is applying for a large family childcare license with a max capacity of 14. Currently The property-owner/landlord consent (LIC9149) is not signed. LPA informed applicant that she will not be able to care for more than 12 children. Residents of the home are the applicant, and her son. Per the applicant, operation hours are Monday- Friday 5:00a.m. -11:59 p.m. Ages of children are 0 (infant 12 years old. Applicant was informed that any changes to ages, hours, days of operation shall be submitted to the department for approval prior. LPA reviewed at the bottom of the Application Applicant/ Licensee Responsibility.The home is a one-story home with 3 bedrooms, 2 bathrooms, patio door, living room, kitchen, & garage. The off limits will be 1 bedroom,#1 bathroom, garage, and patio. LPA observe the following documents for the board, Facility Sketch, Emergency Disaster Plan, Notification of Parents Rights, Earthquake Preparedness. The Main Day care will be in Bedroom #2 and Bedroom #3. Parents will enter through the front door and ring the bell. The Living room will only be used for dropping off and pick up. In the living room LPA observed one couch, table, television, television stand, play& foldaway farmhouse, 3-piece basketball set, three foldable tables, two benches. LPA observed the listening center with headphones, cd’s, cassette tapes, dry erase markers, and books to write in. In the Computer Center there is a computer, keyboard, little art studio. In the living room is a small sliding door with a lock. Once the sliding door is open there are two steps. In the back of the unit is a gated fence this is off limits to the children in care.LPA spoke to the applicant about the safety of the children. All electrical outlets were covered. LPA and applicant tested the fire alarm and carbon monoxide. The fire extinguisher was a 2-A:10B: C. Page1

A First Aid Kit was fully stocked. Per the applicant there no weapons, pets, or bodies of water in the home. At the time of the inspection LPA did not observe any pets, weapons, or bodies of water.Next to the living room the kitchen was observed. In the kitchen area there is a colorful security gate to block off the area between the kitchen and the area where the children will sit down to have their meals. LPA observed two booster seats and highchairs. There is one wooden breakfast nook. kitchen cabinets were observed. The stove, refrigerator, sink and counter space was inspected. Knifes and sharp objects were observed to be inaccessible to the children in care. The home was inspected for safety, comfort, cleanliness, telephone services, heating ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children in care. Applicant confirmed that the facility will be providing breakfast, two snacks lunch, and dinner. LPA discuss food preparation, storage and ensuring a log and information regarding dietary restrictions and allergies are kept up to date.

From the living room is a hallway as you enter the hallway is a closet. LPA checked the closet and observed toys and learning materials for the children. To the left of the closet is Bedroom #1 which will be off limits to the children in care.Bathroom #2 is next to the bedroom#1 and is off limits to the children in care. LPA checked the bathroom. LPA observed in the hallway on the door safety doorknobs. Next to the Bathroom#2 is the Infant room bedroom #2.LPA observed 1 changing table, two cribs, 1 Graco Day 2 Dream Travel Bassinet & Sleep & Play, 1 diaper genie, 1 small blue table with 2 small blue chairs cubbies for the infant’s belongings. For the Graco Day 2 Dream Travel Basinet LPA read the booklet. Travel Basinet & Sleep Play playyard will stop being used when child reaches 35 in. (89cm) in height, weighs more than 30lb(14kg), or is able to climb out whichever comes. In the closet observed on the right side of the door, books, toys, bibs, cloth diapers, t-shirts. On the right side of the closet door LPA observed soft fabric books, wipes, soft toys. In the middle of the closet LPA observed puzzle, blocks, and other educational materials for the children in care. Directly across from Bedroom#2 is Bedroom #3 which is the toddler room. In the toddler room LPA observed a blue table in the middle with 7 chairs, dolls, play kitchen, grocery cart, play iron board, dramatic play toys, puzzles, books, paint, and other craft materials. In the closet LPA observed on the right side of the door educational materials. On the left side of the closet door are all craft materials. In the middle of the closet there are at the top red and blue mats for the children for mapping and more books.

Applicant was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on childcare licensing, and provide many other helpful resources to the licensees and the public. Immunizations: Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the inspection. Mandated Reporter Training: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicant was made aware that state law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category. Applicant was also reminded that only children who are eating may be in highchairs and that car seats are utilized only for transportation.

Safe Sleep webpage at LPA also informed applicant [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was also informed that the provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome.
Applicant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. · In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification and a valid criminal record clearance associated to the facility license.

·Annual fees must be paid promptly and by the due date or a late fee shall be assessed, and/or the License shall be terminated. (If paying by check please make sure to write facility number on check to ensure that payment is applied to your facility number)

· The fire extinguisher must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked, and batteries should be replaced.

· Changes should be reported the to the Department as soon as they occur such as construction and remodeling, telephone number changes and/or if you move from home.

· Reporting requirements: Applicant must report any unusual incident or injuries to the Child Care Regional office by telephone within 24 hours and in writing within 7 days. Applicant was provided with LIC 624 as a reference.

·Fire and safety drills must be performed every six months and documented for review by the Department.

· All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
