32 | During visit LPA Richmond and Lovest observed the following:
1. LPAs observed cribs free from all loose articles and objects.
2. LPAs observed no objects hanging above or attached to the side of the crib.
3. LPAs observed disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children in care
7. S03 has completed 1st aid/cpr on 11/10/24.
4 of 11 Citations issued on 10/28/2024 has been cleared.
LIC421FC has been completed, signed and isssued for the following citations:
5.Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: (3) Infants up to 12 months of age who are sleeping in a position other than on their back
6. Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized
8. employee of a licensed child day care facility shall complete the mandated reporter training
10.Each record shall contain information including, but not limited to, the following: (8) Medical assessment LIC 627
11. Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant
LPA advised director that during absence from facility a qualified assistant can be delegated to complete director responsibilities. LPA provided director with LIC 308 to complete and submit to department.
An exit interview was conducted. A copy of this report, appeals rights, notice of site visit, deficiencies clearance letters were discussed and provided to Licensee. Civil penalties assessed during visit. See LIC421FC.
