25 | On 03/06/2025, Licensing Program Analysts (LPAs) Adrian Risher & Chartrice Johnson conducted a case management deficiencies visit. LPAs observed 4 children in care at the time of the inspection.
On 01/15/2025, LPM Deborah Lowe was advised that the facility has started accepting toddlers before license approval. It has been determined that the facility Is caring for children under 2 years of age prior to obtaining the license and fire clearance. The facility is operating beyond the limitations of the license. This is an immediate health & safety risk for children in care & a TYPE A Limitations on Capacity violation.

Upon receipt of this report, the Licensee/Director shall post the Notice of Site Visit (LIC 9213) and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224) form must be maintained in each child’s file immediately upon receipt from parent. Director was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224).
Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
