On 05/21/2025 Licensing Program Analyst (LPA) Cristina Castellanos and LPA Brittany Lovest arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations. LPAs met with Director Erin Tamashiro and discussed the purpose of the visit. LPAs toured the facility and observed 16 children in care with 6 staff members providing care and supervision.

LPA Castellanos requested the following documents: Children's roster, children’s sign-in & sign-out sheets from 05/01-21/2025, staff roster, staff timesheets from 05/12-16/2025. Additionally, LPAs initiated staff and children’s interviews.

During the investigation at 10:59am LPA Lovest observed staff S1 place child C6 on the crib where a portable sound machine was attached to the side of a crib. LPA Lovest informed S1 that there shall be no objects hanging above or attached to the side of the crib. S1 replied, “Oh this can’t be on the side.” LPA Lovest responded, “No, nothing can be on the sides.”

At approximately 10:00am LPA Castellanos observed six (6) infants and two (2) adult staff members in the infant outdoor play area. LPA Castellanos began to inspect the infant toys located outside in the designated infant outdoor play area. LPA observed the soft blocks to not be clean, when touched dirt residue was transferred to LPA hands and a sticky substance on surface. Furthermore, LPA Castellanos observed the plastic toy container holding the soft blocks to be broken, the toy container is not maintained in good repair and safe condition.

The facility did not comply with the following regulations:
101439.1(f)(3) - Infant Care Center Sleeping Equipment
101439(k)(5) - Infant Care Center Fixtures, Furniture, Equipment and Supplies
101438.1(d) - Infant Care General Sanitation

Based on information obtained and observation, it was found that staff are not providing a safe environment for sleeping infants, facility equipment pose a hazard to daycare children, and facility toys are not clean, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

There were three (3) deficiencies cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information, one (1) Type B and two (2) Type A.



Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

An exit interview and Plan of Corrections were attempted with staff member S7. A copy of this report and appeal rights were left with staff member S7.
