On 9/7/2023 Licensing Program Analyst (LPA) Jillinda Chandler met with Omar Mcgee - applicant and Darrell Pork - director, for the purpose of conducting a pre-licensing inspection for L.A. City Care infant center (197495287), there is a pending preschool application on file (197495288). A tour was provided and all areas, according to the provided facility sketch, were inspected for Health and Safety compliance, per Title 22.
The building is a single story building, 1 room (rm. 3) dedicated to infant care and 2 rooms (1 & 2) dedicated to preschool activities. There is an approved fire clearance on file conducted by Inspector Ashlin Wilburn of the L.A. City Fire Department, Institutions Unit.
The applicant is requesting a license for 15 infants; ages 0-2 years .The child care center will operate Monday – Saturday 6:00 a.m. – 6:00 p.m.

LPA observed the following of the indoor activity space:


Age appropriate furniture and equipment was observed in good repair
A variety of age-appropriate equipment, toys that provide and encourage Auditory stimulation, Visual stimulation, Tactile stimulation, and Manipulative skills.
LPA observed 4 cribs and cots for napping in good condition. The applicant shall devise a plan to isolate the crib area, and add evacuation wheels to a minimum of 2.
The building uses air condition and heating units and windows for ventilation.
Fire extinguisher were observed, last serviced on 3/20//2023.
Carbon monoxide detector were not present during todays inspection
LPAs observed one changing table within arms reach of a sink and padding at least 3 inches thick and raised sides. pg.1

The applicant plans to use potty chairs for potty training children, There shall be 1 potty chair per 5 children in training.
Waste bins were observed with tight fitting lids, for soiled beddings and materials.
First aid kits were available with the required essentials: scissors, bandages, tweezers, and thermometer
There are monitoring cameras throughout the center
Adequate lighting was observed
The classrooms were clean, in good repair
No Fireplaces or open face heaters were observed
Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children, placed in locked cabinet and storage room

FOOD SERVICES
Parents will provide formulas and baby food, foods shall be labeled and stored accordingly. Unused formulas and food shall be discarded immediately after feeding.
Bottled drinking water will be provided
Refrigeration was observed for storage of infant formulas and foods capable of contamination. Applicant shall provide a thermostat to ensure foods are stored at 45 degrees or less.
LPA observed 3 low standing feeding chairs

RESTROOMS

THERE WERE:


1 changing tables, within arms reach of a sink
Applicant shall provide 1 potty chair, per every 5 infants, restrooms located in the center shall be designated to the preschool only or a waiver shall be requested to share the preschool restrooms.

Measurements for the indoor space were:
392.28 divided by 35 sq. ft. per child, for a total of 11 children
OUTDOOR ACTIVITY SPACE
Age appropriate toys and equipment were observed in fair repair.
The play yard gated with a 4 inch or higher gate.
Climbing equipment needs resilient cushioning in fall zones.
LPA observed did not observe shading, or benches for resting. Applicant shall provide shading and benches prior to licensure.
Applicant shall devise a plan to ensure that asphalt areas are free of tripping hazards. Make the bricked area, the crawl space and the side of the south side of the building inaccessible to children.

Measurements for the outdoor activity area were 1335.76 divided by 75 sq. ft. per child for capacity total of 17 children.

Based on today’s inspection the facility shall be recommended for a capacity 11 infants based on the indoor measurements.

The following recommendations were made during todays inspection and shall be provided by prior to licensure, and no later than 10/30/2023:

The applicant shall devise a plan to isolate the crib area and add evacuation wheels to a minimum of 2.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPA discussed the safe sleep regulations with applicanand discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified, of any recalls on their purchased equipment.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
