13 | On 8/14/25, Licensing Program Analyst (LPA), Veronica Wheatley conducted an unannounced inspection regarding the above allegations. LPA arrived to the facility and observed 6 preschool aged children with three staff members (Staff #1, #2, #3). LPA observed the staff members supervising the children in two classrooms which are called the church nursery school classrooms. The two classrooms can be entered street level from Penn Street or from the parking lot on Palm in which stairs are required. LPA observed the children playing inside of the classrooms. These classrooms are not licensed by the Department. The classroom licensed by the Department is downstairs. According to staff, the program operates on Tuesdays and Thursdays 9am to 12pm for one year. LPA observed a copy of the sign in/out sheet which verifies the days and times of operation and the seven names of children enrolled in care. Staff informed LPA that the person in charge is Amy Cruse, however she is not present today. LPA spoke to Amy Cruse on the telephone and informed her that the program is operating beyond the terms of the license and must cease operating today. Amy stated that it was her understanding that they were exempted from licensure. During the inspection, LPA met the Interim Executive Director William Ingram and informed him of the violation and must cease operating immediately. Mr. Ingram informed the parents when they arrived at 12pm that the program is closed effective today.
