On 6/3/2025 Licensing Program Analysts (LPASs) Chartrice Johnson and Jillinda Chandler made an announced visit to World City Center Inglewood located at 633 Aerick Street Inglewood, CA 90301. The purpose of today’s visit was to conduct a pre-licensing inspection. LPAS met with applicant Brittie Crawford. This is an application for a single license for a total capacity of 101; 8 infants ages 0 – 24 months, and 93 preschoolers, ages 2 years until entry into first grade. The days and hours of operation will be Monday-Friday 7:30am-5:30pm. This is a two-story building. The bottom floor contains off limits areas including the kitchen and storage area, as well as a bathroom that is accessible to children during outdoor play. This bathroom contains one sink and one toilet to be used by children during outside play time. The staircase leading to the lower level is inaccessible to children in care. The lower level contains the outdoor play area that is accessible via sloped walkways adjacent to the parking lot.

The facility has a total of 5 classrooms on the street level. Preschool classrooms are #1 (near the front entrance of the building); #2 (on the right side of the corridor near the side entrance from the parking lot); #3 (on the right side of the corridor); and #5 (on the left side of the corridor). Classroom #4 (at the rear of the corridor) will be dedicated to infants. Each classroom has a bathroom which contains 2 toilets and 2 sinks for children. The main level also contains a library, break room, conference room, teacher’s lounge, director’s office, storage room, and two staff bathrooms. There is an approved fire clearance on file conducted on 04/21/2025 by Inspector Judkins of the Los Angeles County Fire Department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed

INDOOR ACTIVITY SPACE

Fire extinguishers were 2A10BC or larger. Applicant was informed that there shall be a Disaster and Mass Casualty Plan readily available for review per Title 22, section 101174 at the center.

Carbon monoxide detectors were observed in each classroom.

First aid kits were available with the required essentials: bandages, ointments, manuals, and thermometers. Applicant shall ensure first aid kits contain scissors and tweezers.

Age-appropriate furniture and equipment was observed in good repair. LPAS observed a combination of toys that provided and encouraged: Auditory stimulation, Visual stimulation, Tactile stimulation and Manipulative skills in the infant classroom.

Cots were observed for napping in good repair in each classroom. LPAS observed 8 cribs in the napping area of the infant classroom. Applicant was informed per Title 22, Chapter 1, Sub Chapter 2, Section 101439 (g), Cribs, mats or cots shall be arranged so as to provide a walkway and work space between the cribs, mats or cots sufficient to permit staff to reach each infant without having to step over or reach over any other infant.

Cubbies and backpack hooks were observed for children’s belongings and infant diapering supplies.

Drinking water will be provided through bottled water service. Children use their personal drinking canisters when drinking water.

The facility has central heating and cooling.

Windows were in good repair free of chipping paint, dirt, insects, or debris. Adequate lighting was observed.

The classrooms were clean in good repair.

The director’s office and staff restroom within the office will be used for isolation of ill children. Applicant shall ensure there will be a mat and a crib or portable crib are readily available for the isolation area.

The facility uses a Google voice phone number which is forwarded to the applicant’s phone. Teachers use the Class Dojo app to communicate with parents and Slack to communicate amongst staff.

Parents and authorized adults will sign children in and out, using the ProCare app. Parents sign children in using a QR code located at the entrance of the facility.

The required postings were also posted. Applicant shall post an emergency exit map with other required postings. Applicant shall post license number on all advertisements and signage.

Measurements for the indoor activity space for preschool was 3112.02 square feet, which will not accommodate the applicant’s request. Measurements for the indoor activity space for infants was 602.19 which will accommodate the applicant’s request.

FOOD SERVICE:

Snacks for the children are provided by the center. Weekly menus shall be posted for review.

OUTDOOR ACTIVITY SPACE

The center has separate areas for each component, preschool will require a waiver to meet Title 22, section 101238.2 (a); There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity.

The outdoor play yards were gated with a 4 foot or higher gate.

Age-appropriate toys and equipment were observed in fair condition. Applicant shall provide a plan for the preschool climbing equipment.

LPAS observed shade and benches for resting on the preschool and infant yards.

Measurements for the outdoor activity area were 710.62 for the infants for a total of 9 which will accommodate the requested capacity. Measurements for the outdoor activity area for the preschool was 5400.05 which will not accommodate the requested capacity.

LPAS observed a kitchen with a sink, storage for foods and refrigerator. Foods and toxins or chemicals shall be stored separately, and LPAS did not observe any expired or contaminated foods. This area was clean, free of insects or vermin.

Center has devised an Incidental Medical Service (IMS) plan to provide to parents of children with allergies (epi-pen), asthmatic (inhalers), and glucose monitoring (diabetics). IMS was discussed with the applicant.

Trash cans used for solid waste were observed with tight fitting lids.

RESTROOMS

One changing table within arm’s reach of a sink was observed in infant classroom. One changing table in classroom #3 was observed to not be in within arm’s reach of a sink. Per applicant, the changing table in classroom #3 will be removed. There were 11 sinks and 11 toilets available for children use, which will accommodate the requested capacity. Per Title 22, section 101439(i)(1)(2) There shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every five infants being toilet trained. (1) If sufficient potty chairs are not available for the number of infants being toilet trained, a combination of potty chairs and toilets with training seats may be used to meet the ratio specified in (i) above.


(2) Infants shall not be permitted to play with potty chairs.

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water.

Based on today’s inspection the facility shall be recommended for a maximum capacity of 96: 8 infants and 88 preschool children determined by the indoor measurements and at the applicants requests, following the recommended corrections, waiver approval and required documents:

· Applicant shall install alerting devices on exit doors


· Applicant shall ensure that rugs do not cause a tripping hazzard.
· Applicant shall create a plan to make the side emergency door near the parking lot inaccessible to children in care.
· Separation for the napping area in the infant room shall be at least 4 feet and sound absorbent
· Applicant shall ensure that poles in room #3 and outdoor area has padding
· Applicant shall provide proof of the working faucets at the sinks in rooms #2 and #3 bathrooms
· The outdoor preschool component requires a waiver to meet Title 22, section 101238.2 (a); There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity.
· Applicant shall provide plan for the preschool climbing equipment to ensure safety.
· Standard toilet for accessible during outdoor play shall have assistance equipment available
· Applicant shall make water hose and HVAC equipment inaccessible to children in care.
· Applicant shall ensure that first aid kits contain tweezers and scissors

The following items were discussed:

The applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAS reviewed with facility representative the LIC 311A, Records To be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the applicant shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

This facility provides Incidental Medical Services – IMS. LPAS reviewed storage of “medication and equipment/supplies.
