On 08/01/2024 at 12:05 Licensing Program Analyst (LPA) Judy Laureano arrived at above mentioned facility for the purpose of investigating the above-mentioned allegations.

LPA was greeted by the Soccer Coach who was in the outdoor area with one staff member, Staff 1 (S1) alone with with 14 children. LPA observed between 12:08 and 12:14, staff1 was left alone supervising 14 children. At 12:15 p.m. Staff 2 (S2) was observed back in the outdoor space.

Based on observation and record review, 14 children were left under the supervision of a teacher for approximalty 8 minutes, there is a preponderance of evidence to prove the allegation of not providing adequate supervision did occur. Therefore, the allegation of Care and Supervision is to be SUBSTANTIAED.

LPA toured the facility and observed 17 children and 3 staff members providing care and supervision. LPA met with Site Director J. Viramontes and discussed the purpose of the inspection. Site Director stated licensee/owner will be at the facility shortly. Licensee/owner arrived at facility at approximalty 12:30 p.m.

LPA requested the following documents: children's roster and parent handbook. LPA Laureano reviewed children’s file and staff files.

LPA reviewed staff files and Guardian and confirmed that Staff 4 does not have a current clearance associated to the facility. Owner confirmed that staff has been employed for more than 5 days. Based on information obtained, there is a preponderance of evidence to prove the allegation did occur. Therefore, the allegation of Criminal Record Clearance is SUBSTANTIAED.

LPA Judy Laureano issued two Type A deficiency during today's inspection (see LIC 9099-D for details).

Effective January 1, 2009, all licensees must comply with Assembly Bill (AB) 978. Assembly Bill 978 requires the assessment of an immediate civil penalty for designated serious violations at community care facilities. Effective January 1, 2007, the licensee must comply with Assembly Bill 633 as follows: Copies of any licensing report that documents a Type A citation - this includes facility visits and substantiated complaint investigations. Copies of any licensing documents pertaining to a noncompliance conference between licensing management and licensees. Copies of a summary of an accusation indicating the Department’s intent to revoke the facility’s licenses. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolling children. The licensee shall keep verification of receipt in each child’s file at the facility as proof of compliance (LIC 9224).
Upon receipt of this report, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
An exit interview was conducted, and report was reviewed with Licensee Sigal Redfield, facility owner.
A copy of this report and appeal rights were discussed and left with Licensee Sigal Redfield, Owner, whose signature on this form confirm receipt of these documents.
