On 06/27/2024 Licensing Program Analysts (LPA) Jillinda Chandler met with Judy Mickens (Applicant) at 7422 Western Ave. Los Angeles, CA. 90047. The purpose of the visit was to inspect the facility for Health and Safety compliance prior to licensure . There were no children or staff members present during this inspection. The applicant is requesting a preschool component for 9 children ages 2 years - entry into first grade, and a toddler component for children ages 18 - 36 months. The childcare center will operate Monday – Friday 6:00 A.M. – 6:30 P.M. There is an approved fire clearance on file conducted on 12/22/2023 by Inspector Ildefonso Felix of the L.A City Fire Department. LPA toured and inspected all indoor and outdoor areas according to the provided facility sketch. This a single story building, the facility has a total of 5 classrooms: Rooms: C (last classroom on the west side of building), the Learning Center (right of the computer room), the Computer Room (next to entry), Room 3 (on the east side of the building) and Room A (toddler room on the north east side of the building).

The following was observed.



Parents and authorized representatives will enter and exit through the entrance gate off of Western Avenue.

Parents and authorized representatives will sign in and out using their original signatures, applicant was informed that all sign in sheets shall be retained for a minimum of 30 days and made readily available upon request.

The facility will use filtered water dispensers for an indoor and outdoor water supply.

Carbon monoxide detectors, smoke and fire extinguishers were operable and within compliance

First aid kits were not observed during todays inspection.

Furniture and equipment were age appropriate and observed to be maintained in good condition free of sharp, loose or pointed parts.

No napping equipment was observed during todays inspection.

Cubbies will be used for storage of children clothing and personal belongings, LPA observed an adequate amount of storage cubbies.

All classrooms are equipped with central air-conditioning and heating, and windows for ventilation . The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris.

Trash cans with tight fitting lids were observed for soiled materials in both restrooms, there was 1 restroom with 3 sinks, 3 urinals and 2 toilets (during todays inspection 1 urinal was observed to be non-operable) dedicated to the preschool and 1 restroom with 2 toilets and 2 sinks (one sink in classroom) for the toddler component.

There is a working telephone located in the directors office



Ill children will use the directors office and the staff restroom for isolation purposes. The isolation area will be equipped with a cot for ill children and shall be physically and visually supervised by staff at all times.

The applicant has developed and implemented a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication is stored in the office in containers inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator inaccessible to children. Applicant shall provide a thermostat for the refrigerator.

In the Kitchen there is a sink and refrigeration. The facility plans to apply for a food program to provide all other meals. Soaps, detergents, cleaning compounds or similar substances shall be stored in a locked storage cabinet, separate from food supplies.

The required postings were posted in the entry area for parents and visitors to review.
The facility shall prepare a disaster and mass casualty plan of action and ensure drills are conducted and recorded bi-annually or quarterly.

LPA did not observe any bodies of water on the premises.

The facility does not plan to provide transportation at this time.

Outdoor Activity Space

LPA did not observe shading or benches for resting on either yard during todays inspection.

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space did not have cushioning an no climbing apparatus during today inspection. Applicant shall devise a plan to make all water valves and spigots inaccessible to children in care. Applicant shall place a gate at the top of the basement stairwell out side of room 3, provide privacy cover on the right side of the back yard, secure the hot water tanks cover and clean toys.

Measurements for the outdoor activity spaces were 1738.64 for the preschool yard (front yard), and 984.41 for the toddler yard (back yard) which will accommodate the requested capacity.

Based on todays inspection the facility shall be recommended for a capacity of 9 preschoolers and 6 toddler children determined by the applicants requested capacity the recommended corrections.



The following was also discussed with the applicant:

The applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
