32 | On 5/08/2026, Licensing Program Analyst (LPA) Ranita Richmond completed an initial 10-day complaint investigation. During inspection, the LPA toured the facility, obtained pertinent documents and interviewed staff.
During LPA record reviews and staff interviews, it has been determined that staff are providing medical attention to children as incidents occur and providing parents with incident reports to serve as documentation of incidents.
Between 05/08/26 and 7/30/26 LPA Richmond contacted parents of children enrolled in the above-mentioned Child-Care Center to conduct interviews. Per parent interviews the parents are pleased with the services being provided by the CCC and have little to no complaints about care provided by CCC.
On 7/30/26 LPA Richmond and Director Maliha reviewed billing and payments. In reviewing the billing and payments, it was noted that Director Maliha offers different billing plans for different service needs. The billing documents indicate that there have been several changes in the services provided. LPA reviewed with director how services are charged to parents. Per director the parent can make changes to the billing plan monthly to fit the needs of services. In this case the parent made changes to the billing plan frequently, at times leaving a balance to the previous billing plan.
A full investigation was conducted which included observations, records reviews, and interviews. Based on observations, record reviews, and interviews, there is not sufficient evidence to show that personal rights or financial issues were violated. Therefore, the above allegations are found to be UNSUBSTANTIATED, meaning that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.
Per Title 22 Regulations and Health and Safety Codes, no citations were issued.
An exit interview was conducted, a copy of this report was read and provided to director Maliha Fahkri.
Notice of Site Visit was provided and required to be posted for 30 days.
