32 | During the investigation, LPA Carus observed/ collected additional information to show that termites are present in the facility which poses/pose a potential health, safety or personal rights risk to persons in care. On 11/13/2025, licensee provided LPA Carus with a pest control treatment receipt showing that the primary termite presence is in the attic, and that professional treatment will begin on 11/15/2025. The licensee agrees to ongoing treatments in order to eliminate the termites permanently.
Additionally, LPA Carus observed laminate floors in 2 areas children have access to in the facility that are weak, and laminate floors in one of the children’s classrooms that are broken. These weak and breaking areas of the flooring are considered to be a potential hazard and poses/pose a potential health, safety or personal rights risk to persons in care.
The licensee has made these rooms with damaged flooring inaccessible to children in care until the floor can be repaired properly. Since 10/29/2025, the licensee has been in contact with the flooring company. These area have been made inaccessible for children & visitors. The licensee has agreed to maintain communication on a monthly basis with LPA Carus until the floors have been repaired.
Based on the evidence & information obtained during the investigation, which included interviews with relevant parties, relevant documentation, and LPA observations, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated.
1 Type-B violation will be cited today.
Exit interview was conducted with Licensee, Mary Mellone and a copy of the report was provided. Appeal rights were reviewed and provided.
