On 12/12/2025, LPA Clayton conducted an unannounced visit to the Light of Knowledge Child Care Center to deliver the findings on a complaint. LPA Clayton was greeted by teachers Salwa Awad. Director Rafeda Shamsa arrived later. LPA Clayton toured the facility for a Health and Safety inspection and observed 8 preschool children in care, being cared for and supervised by 1 fingerprint cleared staff.
On 10/08/2025 ESCCRO received a complaint alleging the following:
• FOOD SERVICE: Staff not meeting daycare child’s dietary needs resulting in allergic reaction.

On 10/09/2025 LPA Clayton toured the CCC inside and outside for a Health and Safety Inspection, reviewed staff and children’s files and interviewed staff. LPA Clayton conducted a thorough investigation which included facility inspections, observations, and staff interviews. LPA interview with Director who acknowledged that on at least one occasion, the child mentioned in the complaint was served corn and it is indicated in the child’s file that she has a corn allergy.

This requirement was not met as evidenced by: The child in care suffered a dislocated elbow in care, and LPA Clayton's observation of teacher S1 pulling children by the wrist, and arm, which poses and immediate Health and Safety risk to children in care.

On 12/12/2025, LPA Clayton conducted an unannounced visit to the Light of Knowledge Child Care Center to deliver the findings on a complaint. LPA Clayton was greeted by teachers Salwa Awad. Director Rafeda Shamsa arrived later. LPA Clayton toured the facility for a Health and Safety inspection and observed 8 preschool children in care, being cared for and supervised by 1 fingerprint cleared staff.
On 10/08/2025 ESCCRO received a complaint alleging the following:

• REPORTING REQUIREMENTS: Staff failed to report injury to authorized representative in a timely matter.

On 10/09/2025, LPA Clayton toured the CCC inside and outside for a Health and Safety Inspection, reviewed staff and children’s files and interviewed staff. LPA Clayton observed the incident report in the child’s file, completed on the date of the incident. In addition, the Director reported the incident to the Department via telephone the day the incident occurred. According to the Director, the staff was intending to call mom, but the child’s speech therapist made the call first.

Based on information obtained during the investigation that included LPA observations, the allegation of Reporting Requirement is found to be UNSUBSTANTIATED, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur.

Exit interview conducted and report was reviewed with Director Rafeda Shamsa. LPA Clayton posted a Notice of Site visit which is to remain posted for 30 days.

Based on information obtained during the investigation that included LPA Claytons observation, it has been determined that facility staff failed to meet the child's dietary needs by serving her food they were made aware that she is allergic to. The preponderance of evidence standard has been met; therefore, the allegations are found to be SUBSTANTIATED. A Type A Violations of the California Code of Regulations, Title 22, 101223 (a) (1) (3) is cited on the attached LIC 9099-D.

Director is to provide parents of currently enrolled children and parents of children enrolled over the next 12 months copies of the Licensing Report containing the Type A violation and maintain a signed copy of the LIC 9224 in the children’s files.

An exit interview was conducted, and this report was reviewed with Director Rafeda, and Appeal Rights were discussed and provided.

LPA Clayton posted a notice of site visit which must remain posted for 30 days.
