32 | According to the narrative provided by S1, S2, and S3, C2 did not show any observable signs of cuts, bruises, abrasions, or ailments for the rest of the school day. P1 provided a narrative indicating that C2 was taken to the emergency room on 6/22/2026 at around 9pm since they had noticed signs of a possible concussion to C2. Professional medical examination indicated that there were no signs of a concussion, or internal bleeding in C2, and the child (C2) was kept home from school for the remainder of the week and since returned on 7/1/2026. The school was not notified of the emergency room visit until 6/29/2026, and the parents of Child 2 have not as of this point provided the note from the doctor indicating the visit that took place on 6/22/2026.
LPA Carus inspected the play area involved in the incident, and observed it to be safe, stable, secure, appropriate for the ages of children enrolled at the pre school, and free of tripping or slipping hazards.
During today’s visit, the director confirmed that since this incident occurred, the school will report all possible injuries/incidents regarding children to their parents on the day they occur as an additional precautionary measure. Furthermore, LPA Carus advised director to inform staff to keep a close eye on children who have been observed to have rough playing habits.
At this time, based on the available information, there is not a preponderance of evidence to indicate that a lack of supervision occurred at the pre school. No regulatory violations will be cited today.
