On 11/04/2025, LPA Clayton conducted an unannounced visit to the Village Tree Preschool to deliver the findings of the above allegations. LPA was greeted by Director Makhaila Causey and Site Coordinator Cindy Renderos. LPA Clayton toured the facility for a Health and Safety inspection, and observed 11 infants and 29 preschool children in care, being cared for and supervised appropriately by 12 fingerprint cleared staff.

On 09/18/2025, LPA Clayton conducted an inspection and interviewed staff, obtained copies of the facility roster, personnel report, electronic student sign-in/sign-out sheets for August and September 2025 and other documents related to the complaint allegations.

Based on LPAs interviews conducted staff, and documents review, the allegation that the teacher in the toddler room was left alone with 9 toddlers, and therefore out of ratio is Substantiated, meaning the preponderance of evidence standard has been met.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, A Type A citation was issued (see LIC 9099D).

Director will provide parents of currently enrolled children and parents of children enrolled over the next 12 months copies of the Licensing Report containing the Type A violation and maintain a signed LIC 9224 the above-mentioned children’s files.

An exit interview was conducted, and this report and Appeal Rights were discussed and provided to Director Makhaila.

LPA Clayton posted a notice of site visit which must remain posted for 30 days.

This requirement was not met as evidenced by: LPA Clayton interviews with staff and Director who acknowledged that the teacher was alone with 9 toddlers who were awake and that was unable to get any assistance for at least 20 minutes.
