On 11/21/2024 Licensing Program Analyst (LPA) Lisa Clayton and LPA Brittany Lovest conducted an announced Pre-licensing Site Inspection for RELOCATION. LPA’s met with applicant Marta Hernandez de Alvarez and her fingerprint cleared daughter/assistant Liliana Perez. There were no children in care. Hours of operation are Monday through Friday 8:30am to 4:30pm. The program will provide snacks, water and milk. Applicant will provide care to children ages 3 months – school age.

LPA Clayton and LPA Lovest toured the home inside and outside for a Health and Safety inspection. The home is neat and clean with heating and ventilation for safety and comfort.

The facility sketch was reviewed and LPA’s confirmed the single family home consists of the following: living room, dining room, kitchen, 2 bedrooms, 1 full bathroom, laundry room, fenced front yard, fenced backyard and detached garage w/ shed.

Applicant has requested the following rooms be ON-LIMITS: the living room/dining room (main day care room), bedroom #2 (infant/toddler nap/playroom), bathroom, laundry room (to access the backyard only) the fenced front yard and the fenced back yard. The isolation area will be in the main day care room or the infant toddler room.

Applicant has requested the OFF-LIMIT AREAS as follows: bedroom #1, kitchen, and detached garage and shed all of which are inaccessible to children in care by child safety gates, closed and/or locked doors and visual supervision.

There are toys and learning materials present at the facility during today's inspection. Furniture and equipment are in good condition and free of sharp, loose, or pointed parts.

There are no pools, ponds or any other bodies of water on the premises. Per licensee, there are no firearms in the home. Any poisons, detergents/cleaning compounds, medication and hazardous items that can pose a danger to children are made inaccessible to children in care.

Applicant was advised to purchase an AC unit cover that can be in place at all times.


The home has a fully charged 2A:10:BC fire extinguisher in the dining room, fully charged 2A:10:BC fire extinguisher in the infant toddler room, working smoke detector/ carbon monoxide detector combo in the hallway and working smoke detector in the infant toddler room. A Fire Clearance was granted by the Culver City Fire Department on November 12, 2024 and there is a pull-down fire alarm system in the laundry room.

The licensee’s Health and Safety training is complete and CPR and First Aid certificate is current and expires on 08/2025. The licensee's mandated reporter training is complete and expires February 2025. The applicant and her assistant are in compliance with the immunization law which pertains to day care providers.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA provided with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted, and a copy of the CA Child Passenger Safety Law poster, and a Sleep Log.

Incidental Medical Services (IMS) policy was discussed and applicant is aware that the IMS plan and corresponding documents are to be forwarded to the department upon completion. IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee is reminded that ALL assistants, volunteers, frequent visitors, or adults living in the home, that are 18 years of age or older must be fingerprint cleared and associated to this facility prior to being in the presence of children in care or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident. Licensee was reminded of the responsibility as a mandated reporter. All forms can be downloaded at www.ccld.ca.gov

The licensee is reminded any structural changes to the home or additions to the child care facility must be reported to Community Care Licensing. Licensee was reminded of Departments inspection authority, with our without any notice.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&R) throughout California.

During the exit interview, applicant Marta confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO profile in FAS.



The following corrections are required prior to licensing:

· Cover the crawl space hole on the south (driveway) side of the home

· Cover the crawl space on the North side of the home

· Cover the plumbing wall access on the South side of the home

· Cover the tankless gas water heater on the South side of the home

· Secure the safety gate behind the detached garage

· Place a lock on the Shed behind the detached garage

· Place a lock on the chimney furnace access door on the North side of the home

The corrections listed above are to be completed no later than Monday 11/25/2024 via Facetime or Zoom call.

An exit interview was conducted, a copy of this report was read and provided to licensee and will be made available to the public upon request. LPA Clayton posted a Notice of Site Visit which is required to be posted for 30 days.
