On 08/05/2026 Licensing Program Analyst, LPA, Tyra Chavies conducted a Case Management - Licensee initiated inspection at the Gutierrez and Corralejo family child care home. Present at the time of inspection was licensees April Gutierrez and Arianna Corralejo, fiancé Joel Musick and licensee Corralejo newborn. Licensee is requesting an increase in the capacity of her family childcare license to care for no more than 14 children. Hours of operation are Monday-Friday 7:00 AM – 5:30 PM. LPA Chavies did confirm the phone number is 310-613-4983.

Community Care Licensing Division received an approved fire clearance on 07/07/2026 for the above facility address. The home is a two story dwelling located within a town home complex. The home includes 4 bedrooms, 2 and half bathrooms, a living room, dining room, kitchen, backyard and detached garage. The town home complex does have a pool area as well as a tennis court which requires a key fob to access. Both applicants will ensure that children are being supervised at all times. Keys and fob to the recreational areas will be stored in a locked drawer. No pets are currently at the facility.
 
Applicants confirmed the following areas are OFF LIMITS to the children in care: bedroom 1, bedroom 2, bedroom 3 and bedroom 4, bathroom 1, and bathroom 2 which are located on the second floor.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Doors will be keep locked during the hours of operations or while children are in care. The kitchen and detached garage will also be off limits and does have a child safety gate in place to ensure inaccessibility. The kitchen observed. Applicants confirmed the following areas are ON LIMITS to children in care: Living Room, Dining room, half bathroom and backyard. Families will enter the home through the main entrance.

Living room are will be used for sleeping and dining room area will be used as the main activity area. Toys, books and appropriate materials were observed to be in good condition and are age appropriate. A working 5A-B-C fire extinguisher was observed inside the main activity space and First Aid supplies is located in closet. Applicants do have a central A/C as well as heat. A stairway was observed that leads to the second floor. LPA did observe a gate separating stairs from sleeping area. LPA reminded applicant to ensure gates remain in place during hours of operations and/or while children are present. LPA observed applicant test the carbon monoxide and smoke detector and it is in good condition.

Applicant confirmed meals and snacks will be provided by facility. LPA discussed the importance of making sure a system is in place where dietary restrictions and allergies are noted. There are firearms and ammunition on the premises. LPA discussed the importance of the firing pins being moved and stored in separate area as well as ammunition. LPA observed Mr. Musick's firing arms and have approved them to be kept in the home. Mr. Musick has stored the firing pins and ammunition (with an exception of one fire arm having a triggering lock attached) in the garage where it will stay during business hours. LPA did observe a community pool on the premises and have discuss with the applicant's the importance of supervision of children.

LPA Chavies discussed Safe Sleep Regulations and resources with applicant. There is one crib or play yard for each infant in care, cribs and play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard. All cribs or play yards shall meet the United States Consumer Product Safety Commission safety standards. Infants are not swaddled while in care. Provider physically checks
on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Infants can be visually observed through an open door if sleeping in a separate room. Individual Infant Sleeping Plan will need to be completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping.

Applicant was made aware that state law prohibits baby walkers, bouncy seats, saucers and any other items that fall into that category. Applicant was also reminded that only children who are eating may be in highchairs and that car seats are utilized only for transportation.

Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the inspection.

On this date, 01/07/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
 
Notice of site visit was given and must remain posted for 30 days.
