On 9/7/2025 Licensing Program Analyst (LPA) Jillinda Chandler and Doris Whitmore made an announced visit to Culver City Christian School for the purpose of conducting a pre-licensing (component addition/single license conversion)inspection. The school is located at 11312 Washington Blvd. Los Angeles, CA. 90066, on the premises of Culver Community Church. LPA met with Jennette Bolden (director) who provided a tour of the facility. The applicant is requesting a preschool license with a capacity of 65 children, ages 2 years – until entry into first grade; 24 toddlers, ages 18 - 36 months, and 24 school age children ages 6 -12 years . Indoor activities will be conducted in rooms; #7 (Toddlers) # 2-6 (Preschool); and # 1 and Lunch room( School-age )
Day Care days and hours will be, Monday – Friday/ 7:00 A.M. – 6:00 P.M. There is an approved fire clearance on file conducted on 6/18/2025 by Noah Piechowski of the Culver City Fire Department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

THE FOLLOWING WAS OBSERVED OF THE FOLLOWING AREAS:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger, last serviced 9/29/2025. Fire Drill last conducted 7/16/2025.

Carbon monoxide detectors were observed in operable condition.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometers.

Age-appropriate furniture and equipment was observed in fair repair.

Cots were observed, applicant was informed that beddings must be removed when cots are stacked.

Cubbies were observed for children’s belongings.

Drinking water will be provided through bottled water and personal drinking canisters.

The facility uses portable heating and cooling units.

Windows were in need of repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

The classrooms were in fair good repair.

Trash cans used for solid waste were observed with tight fitting lids.

No fireplaces were observed, open face heaters were observed with protective covering.

LPA observed disinfectants, cleaning solutions other toxins in an un-occupied classroom locker within reach of children in care, The items were relocated to an inaccessible area during the inspection.

The director's office will be used for isolation of ill children, the children's restroom will be used and sanitized after each use.

The facility was equipped with working telephones in each classroom.

Parents and authorized adults will sign children in and out, using their original signatures. The director was informed that sign in sheets shall be maintained for a minimum of 30 days and readily available upon the request of the departments representatives.

The required postings were also posted in a prominent area for viewing.

Measurements for the indoor activity spaces were: 537.96 square feet for toddlers which will not accommodate the requested capacity; 3644.49 square feet for preschool, which will accommodate the requested capacity, and 862.43 for school age, which will accommodate the applicant’s request;

FOOD SERVICE:

Meals will be provided by parents, and snacks will be provided by the parents.

LPA observed a storage for foods; foods and toxins or chemicals were stored separately. The refrigerator in room 6 will need a thermometer, LPA did not observe any expired or contaminated foods. In the school age classroom the refrigerator will need a lock or be removed.

Center has devised an Incidental Medical Service (IMS) plan and provide to parents of children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing G-tube feeding. IMS was discussed with the applicant.

Toddlers will use one potty chair for every 5 children

The restrooms were equipped with the necessary toiletries, sinks and toilets were operable and in fair repair. Faucets delivered cold water. LPA observed rusting paint in stalls, applicant shall paint stalls to prevent rusting and chipping

OUTDOOR ACTIVITY SPACE

  • Age-appropriate toys and equipment were in need of cleaning.

  • The climbing structure was not age appropriate for preschool and toddlers. The playground structure will need the manufactured sticker or play ground inspection.
  • Toys are in need of cleaning
  • The play yard was gated with a 4 foot or higher gate.
The wall on the back side of the playground structure will need a a privacy screen for children in care.
  • The gate leading to the parking lot needs to have a locking mechanism to prevent children from exiting the play ground, and the gapping at the bottom of the parking lot gate shall be covered
  • The bathroom leading to the out door activity area shall restrict children from exiting when indoors and entering the indoor activity space when outdoors,
  • Poles shall be padded, chains on swing shall be covered to avoid hand injuries,
  • Gate leading on the south end of the play area near the off limit rooms 8 and 9 shall be locked

  • The small square section outside of the small playground shall provide safe access to enter and cushioning
  • The small staircase in small play area needs to be made inaccessible to children
  • Chipping paint on window seals shall be repaired
  • Children bring their own personal water bottles for drinking.
  • LPA observed sail shades for shade and benches for resting.
  • There were no bodies of water observed during todays inspection

Measurements for the outdoor activity areas were 5693.25 for the main yard, 790.32 for the small play area, and 343.9 for the small play area outside room 6 ; for a total capacity of 89 children which will not accommodate the requested capacity.

Based on today’s inspection the facility shall be recommended for a capacity of 143; 15 toddlers, 104 pre-schoolers and 24 school age children, determined by indoor activity space. The applicant shall request a waiver of Title 22, section 101238.2(a) - Outdoor Activity Space; to accommodate the indoor capacity.

Licensing is be pending corrections and/or pending requirements
