On 12/11/2025 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to USC -Tom Bradley Child Development Center located at 3875 Dublin Ave. Lamert Park, CA. 90008 for the purpose of conducting a pre-licensing inspection. The center is located on the campus of Tom Bradley Elementary School.There were 5 rooms dedicated to preschool activities: rooms 3 and 2 will be designated to the toddler program, room 6 will be dedicated to preschool and rooms 32 and 33 (divided bungalow) shall be dedicated to the infant program.
The applicant is requesting an infant program with a capacity of 16 infants, ages 6 -18 months; a toddler program with a capacity of 32, ages 18 - 36 months, and a preschool program with a capacity of 20 children, ages 2 years until entry into first grade. Daycare will be conducted Monday – Friday/ 7:30 A.M. – 6:00 P.M. There is an approved fire clearance conducted on 7/15/2025 by Harold Woods of the L.A. City Fire Department.
LPA met with Yasmirth Chavez (director) and Kenicia Mota (site supervisor) who provided a tour of the center.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger.

Carbon monoxide detectors were observed in operable condition in each classroom, the school has an integrated smoke and fire alarm system.

First aid kits were available with the required essentials: scissors, bandages, tweezers, antiseptics, manuals and thermometer.

Age-appropriate furniture and equipment was observed in good repair.

Cots were observed for napping in good repair, the director was informed that napping materials shall be removed and stored separately when cot are stacked. LPA observed 8 cribs that met the United States Consumer Product safety standards. The director was provided Safe Sleep regulation for review.

LPA observed low/wide based feeding chairs for infants, infants that are not able to sit shall be fed by staff.

Cubbies were observed for children’s belongings and infants diapering needs.

Drinking water will be provided through filtered water dispensers.

The facility has central heating and cooling. No Fireplaces or open face heaters were observed during todays inspection.

Windows were in fair repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

LPA observed trash cans used for solid waste with tight fitting lids.

The classrooms were clean in good repair. Rugs and floors were clean.

Disinfectants, cleaning solutions, toxins or other poisons were made inaccessible to children in care stored in locked cabinets.

The children will isolate in their designated classrooms, and children's restroom will be used and sanitized after every use.

The facility was equipped with working telephones inside the classrooms

Parents will enter the school using the entry on the north side of the center's outdoor activity area. Parents and authorized adults will sign children in and out, using their original signatures, sign in sheets will be located in each classroom The applicant was informed that sign-in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the department's representatives.

The required postings shall be posted in a prominent area for viewing. LPA observed the required postings: Personal Rights form (LIC 613A). Menus, Child passenger restraint system poster (PUB 269) completed with local health department phone number, Daily activity schedule, Facility Sketch (LIC 999), and Parent’s Rights Poster (PUB 393).

FOOD SERVICE:

Meals will be provided by the center. Meals are delivered from the centers meal vendor. Applicant was informed there shall be an adequate supply of emergency foods readily available in the case of unforeseen circumstances.

LPA observed a prepping area in room 6 a near by a sink, storage for foods and a refrigerator, warming cabinet and a microwave. Foods, toxins and chemicals were stored separately, and LPA did not observe any expired or contaminated foods. Refrigerators and food storage was also observed in the infant rooms.

Center has devised an Incidental Medical Service (IMS) plan and for children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing G-tube feeding. IMS was discussed with the applicant.

RESTROOMS

THERE WERE:

Changing tables within reach of a sink were observed in the toddlers and infant classrooms, the center shall use potty chairs for toddler, the applicant shall provide 1 potty chair per 5 children to accommodate potty training children. Potty chairs must be emptied and cleaned after each use and children must not be allowed to play with potty chairs at any time.

3 sinks, 4 toilets were available for preschool children. The center has exclusive use of the restroom located outside of room 6. The center shall provide postings identifying the restroom as preschool use only and the door leading to the playground shall remain locked at all times.

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in good condition.

The outdoor activity space was gated with a 4 foot or higher gate.

Water shall be readily available for an outdoor activities.

LPA observed shade and benches for resting.

LPA did not observe any climbing apparatus during this visit. Space under the utility boxes located in the outdoors area shall be made inaccessible to children in care. applicant will provide photos of the corrections.

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water and water tables must be removed or emptied immediately after each use.

The center is requesting a waiver of Title 22, section 101238.2(a) Outdoor Activity Space, for the infant, toddler and preschool components to share the same outdoor activity space on alternating schedules.

Indoor measurements were: infants;775.36 square feet for a total capacity of 21 infants (rm. 32= 11 and 33 =10), toddlers; 1418.68 for a total of 40(rm 3=20 and 4= 20 children) and preschool 661.19 (rm. 6= 18 children).

Measurements for the outdoor activity space = 2735.64 for a total capacity of 36 children.

Based on today’s inspection, the facility shall be recommended for a capacity , 18 preschool children determined by measurements of the indoor activity space 16 infants and 32 toddlers based the requested capacity, following required corrections and waiver approval of Title 22, section 101238.2(a) - Outdoor Activity Space, 101438.2(b) Infant Outdoor Activity space, including Written Directives For The Care of Toddlers In Licensed child care centers section. 101804(e).

An exit interview was conducted and this report was discussed and will be emailed to Yasmirth Chavez - director.
