On 10/3/2025 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to The Playgroup House at 1249 26th Street, Santa Monica, CA. 90404, for the purpose of conducting a pre-licensing (change of ownership) inspection. The center is a single story, converted duplexed building with two separate buildings (front and back), day care activities will be conducted in both locations. LPA met with Cassidy Boesh (director) who provided a tour of the facility. The applicant is requesting a preschool license with a capacity of 31 children, 2 – 5 years of age. Day care operations will be conducted Monday – Friday/ 8:00 A.M. – 12:30 P.M.; staff available Monday - Friday/8:00 A.M. - 2:30 P.M. There is an approved fire clearance on file conducted on 8/6/2025 by Armando Reyes of the Santa Monica Fire Department

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Carbon monoxide detectors were observed in operable condition. LPA observed a hard wire fire alarm system.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, thermometer and manual.

Age-appropriate furniture and equipment was observed in good repair.

The center operates an half day program and children do not nap. LPA observed clothe mats that were available for children in need of resting. LPA informed the director that the clothe mats did not meet regulations per Title 22 , section 101239.1

Cubbies were observed for children’s belongings.

Drinking water will be provided through filtered water pitchers.

The facility has heating and cooling units in both classrooms.

Windows were in good repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

The classrooms were clean in good repair.

Trash cans used for solid waste were observed with tight fitting lids.

No Fireplaces or open face heaters were observed.

Disinfectants, cleaning solutions, toxins and other poisons were made inaccessible to children in care.

Ill children will isolate in the tiny room (near the staff's restroom) and use the restroom designated for childrens use with disinfecting and sanitizing after every use.

The facility was equipped with working telephones.

Parents and authorized adults will sign children in and out, using an electronic devise (Playground). The director was informed that sign in sheets must be maintained for a minimum of 30 days and readily available upon the departments request.

The required postings were also posted in a prominent area for viewing.

Measurements for the indoor activity spaces were 643.96 square feet in the front classroom and 599.61 square feet for the back classroom which will accommodate the applicant’s request.

FOOD SERVICE:

Meals will be provided by parents. Director was informed there shall be an adequate amount of emergency foods available for children in the case of an emergency.

LPA observed a kitchen, with a sink, storage for foods and a refrigerator with a thermostat, foods and toxins or chemicals were stored separately, and LPA did not observe any expired or contaminated foods.

Center shall devise an Incidental Medical Service (IMS) plan and provide to parents of children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing G-tube feeding. IMS was discussed with the applicant.

RESTROOMS

THERE WERE:

4 sinks and 4 toilets available for children use

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in good condition.

The play yard was gated with a 4 foot or higher gate.

Sand and grass were observed for cushioning

Water was readily available for an outdoor water source; children use their personal water bottles for drinking.

LPA observed shades and benches for resting.

The space behind the playhouse in the back yard shall be made inaccessible to children in care. This can pose a supervision concern.

There were no bodies of water observed during todays inspection. Director was informed all standing water shall be discarded after every use,

No hazardous conditions or equipment were observed during today’s inspection.

Applicant was informed that the facility number must be published on all advertisements.

Measurements for the combined outdoor activity areas were 4725.05, which will accommodate the requested capacity.

Based on today’s inspection the facility shall be recommended for a capacity of 31 children determined at the request of the applicant and pending recommended corrections.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
