On 03/10/2026 Licensing Program Analysts (LPA) Laticia Thompson and Dawn Dowling met with Christine Refuerzo, Director. The purpose of today’s announced visit is to provide the Director with a Facility Evaluation Report of the Prelicensing Visit conducted on 03/09/2026. The application is for a Single License with a Total capacity of 118; 48 Preschool children ages 2 years - Entry into 1st grade and School Age Children ages 5 - 14 years. The child care center will operate Monday – Friday 6:00 a.m. – 6:00 p.m. The facility has a Private School (Islah Academy) on the premises that operates during the hours of 9:00 am-3:00pm.

On 03/09/2026 LPA Laticia Thompson measured and inspected the on-limits areas. The preschool and school age has a total of 6 classrooms. Classrooms A and B will be dedicated to Preschool. Classrooms 1-4 will be dedicated to School Age. Preschool children will have access to 2 sinks and 3 toilets. School age children will have access to a total of 4 sinks and 4 toilets. School age restrooms are located on the first and second floor.

Preschool Indoor measurements are:

Indoor activity measurements contain a total of 1,005.62 square feet which will not accommodate Applicant’s request for 48 Preschool children. Based on today’s measurement the facility capacity limit for indoor space is 28 Preschoolers

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Preschool Outdoor Activity measurements are:

Outdoor activity space measurements contain a total of 1,361.07 square feet, which will not accommodate Applicant’s request for 48 Preschool children. Based on today’s measurement the facility capacity limit for outdoor space is 18 Preschool children.

School Age Indoor measurements are:

The measurements contain a total of 1,472.23 square feet which will not accommodate Applicant’s request for 70 School Age Children. Based on today’s measurement the facility capacity limit for indoor space is 42 School Age Children

School Age Outdoor Activity measurements are:

Outdoor activity space measurements contain a total of 6,970 square feet, which will accommodate Applicant’s request for 70 School Age Children

The following was observed:



Parents will enter the facility on Madden Street during drop off and pick up by signing children in and out manually. The outdoor activity areas will not be utilized during dismissal hours (3:00 – 3:30pm) of private school .

Per Health and Safety Code 1596.841 Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.



Staff restroom is located next to the school age children’s restroom. The facility has 2 filtered water dispensers located on the first and second floor. The dispenser will be used with disposable cups or to fill children personal water bottles for outdoor and indoor activities. There are operable carbon monoxide detectors in Classrooms A, B and in the middle of the hall way on the second floor. LPA observed fire extinguishers with an inspection date of 08/16/2024. LPA advised director to have fire extinguishers inspected. LPA observed First Aid Kits with missing required materials.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There is a sufficient number of cots for napping and cubbies for storage of children’s clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean and have adequate lighting. There are classroom windows and walls that need repairing of chipping paint. LPA observed trash cans without lids. The facility is equipped with landline telephones service. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in areas inaccessible to children.

The are toilets and handwashing facilities maintained in safe and sanitary condition with an exception of one toilet located in the School Age children’s restroom on the first floor. Hot water temperature controls are maintained within regulation.

The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the overflow room . The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. A specific stall in the preschool restroom will be designated for children who are ill when needed. School Age children will use Staff restroom when ill. The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the office area inaccessible to children. Medication that requires refrigeration will be stored in a lock box and placed in the staff refrigerator inaccessible to children.



The program does not provide lunch and snacks. The required postings are posted next to the entrance on Madden ave for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861
Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Play equipment and materials used by children are age-appropriate. Applicant will place cushioning below slide equipment to prevent injury from falls. The outdoor activity space for preschool and school age children are in need of repair. LPA observe some hazards in the outdoor activity area. Applicant will install shade in the outdoor activity area. LPA did not observe any bodies of water.

The following items were discussed:



The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with director the LIC 311A, Records to be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.

The following corrections are needed before licensure will be considered.

Install plug protector on plug outlets in School Age Classrooms
Install cover plates, repair window, walls with chipping paint in all classrooms and cubbies in hall.
Repair toilet in School Age restroom on 1st floor and damaged wall in classroom #2
Place a lock on Storage closet. Repair floor and remove rug tripping hazards. Repair window stucco in Room #4
Create a pathway for preschool children to access the restroom and prevent commingling with private school children.
Replace trash cans that do not have lids
Place a notice on gate with scheduled times will be open.
Preschool Outdoor; make adjustment to cushioning platform, repair gravel area that is not leveled. Cover water spigot and mail box with cushioning. Place barrier around fire department spout area. Place a barrier to separate school age outdoor activity space. Place barrier between Madden ave side of the building and wooden gate. Place a mechanism where opening is next to the mail box.

School Age outdoor space; enforce chain link fence from bending, close gap between gate opening, relocate trash bins, trim bushes in wall planters and repair graveling and leveling of ground near chain link fence.

Submit waiver for alternating usage of preschool outdoor activity space.

Corrections are due by 04/06/2026.

An exit interview was conducted with plan of corrected items discussed. LPA provided the Director with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
