On 10/23/2025 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to Carousel Preschool and Infant Center for the purpose of conducting a pre-licensing (change of corporation) inspection for 7899 LaTijera Blvd. Ste. 100, Los Angeles, CA. 90045 The center is a two-story building with two infant classrooms; rooms 1 and 2, and 4 preschool classrooms # 3 - 6. LPA met with Jane Uy (Senior Director) and Gina Priatto (Vice President) who provided a tour of the facility. LPA toured the upper level of the building, per Uy the occupants of the upper level are a separate entity. LPA observed multiple rooms offices / activity rooms, restrooms and a kitchen, no day care activities were observed in this area. The applicant is requesting a preschool license with a capacity of 81 children; 30 infants ages 6 weeks - 24 months, and 51 preschool children ages 2 years – until entry into first grade. Daycare will be conducted Monday – Friday / 6:45 A.M.– 5:30 P.M. There is a fire clearance on file approved on 8/19/2025 by Gabriel Lopez of the L.A. City Fire Department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger, last inspected 4/8/2025

Carbon monoxide and smoke detectors were observed in operable condition.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometer.

Age-appropriate furniture and equipment was observed in fair repair.

Cots were observed for napping children that did not require a crib. LPA observed 10 cribs in the napping area, applicant was informed that beddings must be removed from mats and stored separately when mats are stacked, Placement of cribs, mats or cots shall not hinder entrance or exit to and from the napping space. LPA advised the director that the cribs space did not allow access to the infants properly, LPA reduced the allowed number of cribs to 8; meaning there shall only be 8 infants enrolled that are required to sleep in cribs per Title 22, section 101439.1(c) at any given time.

Cubbies and back pack hooks were observed for children’s belongings.

Drinking water will be provided through filtered water dispensers.

The facility has central heating and cooling. No Fireplaces or open face heaters were observed during todays inspection.

Windows were in good repair free of chipping paint, dirt, insects, or debris and adequate lighting was observed.

The classrooms were clean in fair repair.

LPA observed trash cans used for solid waste with tight fitting lids.

Disinfectants, cleaning solutions, toxins or other poisons were observed inaccessible to children in care.

The director’s office and children's restrooms will be used for isolation of ill children.

The facility was equipped with working telephones.

Cameras were observed indoor and outdoors. Per the director cameras are only for monitoring and footage is not retained.

Parents and authorized adults will sign children in and out, using their original signatures. The applicant was informed that sign-in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the department or its representative.

The required postings were also posted; applicant was advised that the postings shall be posted in a prominent area for viewing and updated to reflect the current address of the licensing office, 300 Continental Blvd. Ste. 290A, El Segundo, CA. 90245 ensuring the latest revisions are posted.

FOOD SERVICE:

LPA observed a prepping area, with a sink, storage for foods and a refrigerator with a thermometer, foods and toxins or chemicals were stored separately, and LPA did not observe any expired or contaminated foods. Infant foods, bottles and cups were labeled with children's names.

Meals will be provided by parents and the center shall provide snacks, snack menus were posted. Applicant was informed there shall be an adequate supply of emergency foods readily available for all programs in the case of unforeseen circumstances. The center shall request or provide extra provisions for infants.

LPA observed multiple wide based highchairs, seats and backrests were made of a washable and moisture resistant materials.

THERE WERE:

LPA observed 5 sinks and 4 toilets and 2 urinals available for preschool children use and 4 toilets and 4 sinks for infants, equipment was standard size, LPA observed potty chairs, accessibility adapting stools to assist children when using standard toilets and sinks. LPA observed changing tables with-in arms reach of sinks.

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold and warm water.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in fair repair

The outdoor activity space was gated with a 4 foot or higher gate with intercom entrance.

Sand was observed for cushioning, per the director the sandboxes are maintained daily by staff and grounds keeper.

A water faucet was readily available for an outdoor water source.

LPA observed sail shades for shade and benches for resting. The asphalt yard was in need of shade and cushioning added to parking poles. LPA observed the asphalt area to double as an access area for transportation pickup for the program on the upper level. LPA discussed possibly making this area inaccessible to children in care due to the dual use of the area, LPA will consult with management prior to licensing.

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water should be removed immediately after each use.

MEASUREMENT WERE:

Measurements for the indoor activity spaces for preschool were:


1803.45 square feet, which will accommodate the applicant’s request.
972.27 square feet for the infant program which will not meet the applicants request.
Measurements for the outdoor activity areas were: preschool 6067.31 which will accommodate the requested capacity and infants 982.67 which will not accommodate the applicants request.

Director was advised to review ,refer and adhere to Title 22, articles 6 and 7, and sub-chapter 2, regarding child care regulations of preschool,and infants

Based on today’s inspection, the facility shall be recommended for a capacity of 51 preschool children and 27 infant children determined by the measurements of the indoor activity space.

Applicant shall request an outdoor waiver to meet the infant outdoor activity space requirements; Title 22, Section 101238.2(a)

An exit interview was conducted and a copy of this report was discussed with Jane Uy and Gina Priatto. Report will be emailed to the facility.

Criminal Record Clearance - Child Care Centers facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC TESTING DEADLINE has not passed. LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. PIN 22-05-CCP Page Four

Safe Sleep - LPA discussed the safe sleep regulations with or facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA discussed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication. MyChildCarePlan.org –Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
