On 04/09/2026 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to Bright Beginnings Preschool, located at 2210 Lincoln Blvd. Venice, CA. 90066 on the campus of Venice United Methodist Church. The purpose of todays visit was to conduct a pre-licensing inspection. The center is a single-story building with 4 classrooms. Classrooms 1,2,& 4 will be dedicated to preschool activities; and room 3 will be dedicated to the toddler program. LPA met with Jennifer Shah (applicant) and Jing (Gennie) Xu (director) who provided a tour of the facility. The applicant is requesting a maximum license capacity of 30 children; 12 toddlers ages 18-36 months an 18 preschool children ages 2 - until entry into first grade. Daycare will be conducted Monday - Friday; 8:00 a.m. - 5:00 p.m. There is a fire clearance on file approved on 9/4/2025 by Gabriel Lopez of the L.A. City Fire Department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Fire extinguishers were 2AB10C or larger last inspected 8/26/2025

Carbon monoxide and smoke detectors were observed in operable condition.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometer.

Age-appropriate furniture and equipment was observed in good repair. LPA observed toys for toddlers that provided auditory stimulation, visual stimulation, tactile stimulation and manipulative skills. Although classroom 3 was not equipped with furniture and equipment; this room shall be arranged and equipment provided for day care activities.

Cots were observed for napping; applicant was informed that bedding must be removed and stored separately when cots are stacked.

Cubbies were observed for children’s belongings.

Drinking water will be provided through purchased water using portable dispensers.

Adequate lighting was observed.

Windows sills were in need of cleaning ensuring that they are free of chipping paint, dirt, insects, or debris.

The facility has a heating and cooling system. LPA did observed a a fireplace, the fireplace was non-operable and did not pose a safety hazard, the applicant shall lock the crawl space next to the fire place. there were no open faced heaters observed.

LPA did not observed trash cans used for solid waste with tight-fitting lids.

Disinfectants, cleaning solutions, toxins or other poisons were observed inaccessible to children.

Children will isolate inside their classroom away from other children when ill, the children's restroom will be used with sanitizing and disinfecting after each use. Applicant was informed that there shall be a cot, mat, or crib readily available for ill children and that these children must be under direct visual supervision at all times.

The facility was not equipped with a working telephone. The applicant shall devise a plan for communication during day care operations.

The classrooms were in good repair, applicant shall ensure that the school is dusted and free of insects (spiders) before children are present. Also in rooms 1 and 3 rust stains under sinks shall be cleaned

Parents and authorized adults will sign children in and out, using their original signatures. The applicant was informed that sign-in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the Department, or it's representative.

The required postings were posted; applicant was advised that the postings must be posted in a prominent area for viewing at all times during day care operations. Applicant was informed that all advertisements must contained the facility number.

FOOD SERVICE:

LPA inspected the kitchen located within the building, LPA observed expired insects in the refrigerator and vermin sightings in cabinets and along the walls. Per the applicant the kitchen will not be used for preparing or storage of foods, for daycare children. Meals will be provided by parents and the center will provide snacks. Applicant shall provide verification of an ongoing extermination contract. Applicant was informed there must be an adequate supply of emergency foods readily available in the case of emergencies.

Center shall devise an Incidental Medical Service (IMS) plan and provide to parents of children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing gastrostomy tube (g-tube) feeding. IMS was discussed with the applicant.

3 sinks and 2 toilets available for children use
The restroom was clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. LPA recommends that broad based steps be provided to assist children when using the standard sized toilets. Faucets delivered cold water. Changing tables were within reach of a sink, LPA did not observe adequate padding surfaces that must be at least one inch thick, and covered with washable vinyl or plastic with raised sides at least three inches high. Applicant was informed there shall be a written toilet-training plan for toddlers being toilet trained.

Applicant was informed that children from different programs must always be supervised when using the shared restroom by their assigned staff member or an qualified aide. The staff shall not use restrooms dedicated for preschool use, per the applicant, staff are to use the restroom located across the auditorium, applicant was reminded that teacher/child ratios must be maintained at all times, including restroom breaks.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in fair condition.

LPA observed shading and benches/chairs for resting.

The outdoor activity space was enclosed with a 4 foot or higher gate/wall.

Wood chips and sand were observed for cushioning, in fair condition, per applicant these areas will be maintained by staff.

Water will be made readily available for an outdoor activities.

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water should be removed immediately after each use.

MEASUREMENTS:



Preschool measurements equal 863.20 square feet, which will accommodate the requested capacity.

Toddler’s measurements equals 276.84 square feet, which will not accommodate the requested capacity.

Based on today’s inspection, the applicant decided to request a new capacity based on the indoor measurements, and reconfiguration of the assigned classrooms. Applicant shall provide a updated LIC. 200A (rev.1/25) and a new fire clearance will be updated to the fire department for review.

The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided:

LPA reviewed with applicant the LIC 311A, Records to be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.
